Medical Summary Scan : positive 4A12053233 Sex: male neutered S: Tolerates all handling, coughing continuously during exam O: BAR-H, BCS 6/9, MMs pink and moist, CRT < 2 sec EENT: Severe tartar and gingivitis. Nuclear sclerosis or cataracts OU (ophthalmic exam not performed). No ocular, nasal or aural discharge H/L: NSR, no murmur. Coughing continuously during exam, harsh lung sounds on R side of chest. Abd: Tense (coughing) M/S/I: Amb x4, long nails, long fur coat with some mats, 1 cm dia crusty scab on dorsal abdomen, some feces in fur around anus U/G: Neutered male A: 1. Severe dental disease 2. Coughing – R/O collapsing trachea (vs. heart disease vs. CIRDC vs. chronic bronchitis) 3. Long nails and fur coat with some mats and one scab 4. Nuclear sclerosis or cataracts P: 1. Monitor in Medical for the day to see if coughing decreases after excitement of exam and being moved into a new cage wanes – if not, rec send to BACC for chest rads 2. Permanent placement should include veterinary care for dental disease, as well as grooming PROGNOSIS: GOOD to FAIR 1088
