Medical Summary S/O: PATIENT PRESENTED IN LATERAL RECUMBENCY IN CARDBOARD BOX – FOUND OUTSIDE. INAPPETENT. LATERALLY RECUMBENT ON PRESENTATION BUT BAR VERY WEAK MM = PINK, SL. TACKY HYDRATION = DIFFICULT TO ASSESS DUE TO EMACIATION; LIKELY DEHYDRATED. VERY THIRSTY INTEG = OVERGROWN NAILS X 4. DIRTY COAT WITH AREAS OF MATTING OVER ALL FOUR LIMBS SECTIONS OF DORSUM, TAIL; DRY, FLAKING SKIN OVER DORSUM. SOFT WART-LIKE MASS (APPROX. 2-3 CM IN DIAMETER) OVER CAUDOLATERAL RHL, JUST ABOVE THE STIFLE. EENT = NUCLEAR SCLEROSIS OU, MILD BROWN WAXY DEBRIS AU; NND; MODERATE PERIODONTAL DISEASE, MALOCCLUSION – UNDERBITE H/L = GRADE IV -V/VI PANSYSTOLIC HEART MURMUR, HEARD LOUDEST ON THE LEFT. MILDLY INCREASED BV SOUNDS ON RIGHT SIDE ABDOMEN = SNP, THICK LOOPS OF INTESTINES PALPATED, RECTAL EXAM NOT PERFORMED LN = WNL MS = NON-AMBULATORY, HOWEVER PATIENT IS ABLE TO MOVE LIMBS. BCS: 1/9 NO OBVIOUS FRACTURES, SEEMS TO HAVE GOOD RANGE OF MOTION CNS = A+A. GU = SPAYED FEMALE, NO MGTS A: DIRTY COAT MATTED OVERGROWN NAILS HEART MURMUR DENTAL DISEASE PX: FAIR WITH APPROPRIATE TREATMENT P: AWAITING CONFIRMATION ON LEGAL STATUS, THEN WILL PROCEED WITH PHOTOS, GROOMING, SUPPORTIVE CARE, ETC.
