05/28/2015 Exam Type RE-EXAM – Medical Rating is 3 NC – MAJOR CONDITIONS NOT CONTAGIOUS, Behavior Rating is NONE, Weight 9.8 LBS.
S/O: Geriatric exam – patient is BAR on presentation. Tolerable for handling – growls but cooperative overall. hydration = adequate mm = pink, moist, CRT<2sec. Integ = symmetrical linear areas of hair loss over the cranial aspect of the shoulders and over the thoracic region – measure about 1-2 cm in width. EENT = nuclear sclerosis, OU; clean ears AU, nnd, mild to moderate tartar H/L = nma, nsr, pss; clear lung fields bilaterally Abdomen = tense, non-painful; nmp. Rectal exam reveals soft stool – no masses, empty anal sacs LN = wnl CNS = appropriate MS: BCS = 6/9; bilateral luxating patellas (grade 2/3), no muscle atrohy, no lamenesses noted. A: geriatric MPLs Hairloss areas – likely from chronic use of harness. dental tartar Px: fair to good P: No treatment indicated at this time. Continue to monitor while at BACC
05/27/2015 PET PROFILE MEMO
Basic Information Toris is a 12 year old black and white long haired male chihuahua. He was brought into QAC by his owner of 12 years (since Toris was 6 weeks old) after moving to a a new residence where they could not keep Toris. The client reported that Toris suffers from anal sacks, in which his anal area will become infflamed and filled with fluid. The owner reported that usually Toris will scoot on the floor, which is an indicator that a vet visit is needed to drain the fluid. Toris’s last visit to the vet was last year. Socialization Toris is usually fearful around strangers, as evidenced by hiding or excessively barking. He has mostly been around adults, and is described to be friendly around adults. He likes to play rough with adults, and has not had exposure to other animals or children. The owner reported that Toris was in contact with one other dog, and appeared to be very playful chasing that dog. Toris has never bitten another animal but bit the owner’s cousin last year when he was being collared. The bite did not break the skin. Behavior The owner reported that Toris often goes to the bathroom inside the home, including defacating and urinating. Toris is trained to go outside as well as on wee-wee pads; however, will go to the bathroom on the floor next the pad. Toris has also ben described to bark excessively. He doesn’t appear to be bothered during storms. Toris growls at various situations, including when toys are taken away from him, when he is brushed, and when he is pushed off of furniture. Toris is known to gaurd his food. Additionally, he has struggled when groomed. For A New Family to Know Toris is playful and sweet but likes his space. Toris can be somewhat active when playing with stuffed toys. He love walks, playing chase, and getting his belly rubbed. When at home with his owner, Toris will remain in his designated spot in the home. The owner reported that Toris is well-behaved when left at home alone but whines upon the owner’s return. Toris has never been off of a leash, as the owner was fearful he would run away. On the leash, he is known to pull sometimes. Toris responds to the commands of sit and come, as well as kisses. He eats chicken and steak twice a day. Toris was timid during the intake and spent the entire intake being held in his owner’s arms. He growled and remained whale eyed when approached by the counselor for collaring. The client completed all handling.

05/29/2015 BEHAVIOR EVALUATION – NH ONLY
Exam Type BEHAVIOR
Toris previously lived with one adult and it was reported by the owner that Toris is usually fearful around strangers. He is described as hiding or excessively barking around strangers, and has only really been around adults most of his life. Toris does come to us with a previous bite history when biting one of the owner’s family members without breaking skin. We do not know the specifics of how the bite occurred but due to this information we believe that Toris would do best in a home with no children. The owner also stated that Toris has a tendency to growl when any resources are taken away from him. Toris can be seen at the back of the kennel with stiff and fearful body language when attempting to take him out of his kennel. This behavior was maintained when entering the behavior assessment room. Not all tactile items were able to be conducted as Toris bared his teeth and growled at the assessor on the Look item. He displayed no interest in any of the resource items during this portion of the assessment. When on leash for a dog interaction with another dog on leash Toris does not approach, but instead turns his body to the side and exits. In light of this behavior we feel that placement with a rescue who can provide positive reinforcement-based behavior modification is the best option at this time. Look: 5. Dog bares teeth and growls. Food: 1. Inconclusive dog is not eating. Toy: 1. No interest. Rawhide: 1. No interest. Dog-dog: 2. Dog does not approach the helper dog. Turns body to side in relation to other dog and exits.
