10/16/2015 PET PROFILE MEMO
10/16/15 17:53 Basic Information Sparky is a 10 year old neutered German Sheppard that came in with litter mate Sheila. Owner stated that he could no longer keep them because he is moving out of state and his new place doesn’t allow pets. He got them as a gift from a friend when they were puppies and has had them since. Sparky has no known injuries or health problems and his last vet visit was in 2013. However, he will undergo a complete medical examination. Social Life & Personality Sparky does not like children. He has no experience playing with cats and is respectful to other dogs but does not play or interact with them. He is house-trained and is kept outdoors only. He has a high activity and owner described him as affectionate, independent, mellow and confident. Behavior Issues Owner stated that sparky has no behavior issues and has never bitten or scratched another person or animal. He isn’t bothered with having his food bowl touched or having objects or toys taken from him. He is friendly during baths and isn’t bothered when owner restrains him. For a New Family to Know Sparky knows the commands sit and stay. Fore exercise, he receives play time in the yard only and plays with dog bones for toys. He does not receive off leash walks and the yard is completely gated off so he doesn’t come in contact with passersby. When left alone in the yard he is well behaved and plays with Sheila. He eats dry food only and sleeps in the yard.

10/20/2015 BEHAVIOR EVALUATION – NH ONLY
Exam Type BEHAVIOR
Helper dog: A1054927 Sparky previously lived with 1 adult and another dog (German shepherd). The owner stated Sparky does not like children, but is respectful to other dogs and do not play or interact with them. The owner mentioned Sparky is house-trained, well behaved when home alone and has a high activity level. The owner describes Sparky as affectionate, independent, mellow and confident. At intake it was reported Sparky was nervous and barking in the lobby, Sparky was muzzled and not handle during this time. During SAFER Sparky came into the assessment room nervous and anxious, not all handle items were able to be conducted as Sparky freezes and lip curls at the assessor on the Look item. He displayed no interest in any of the resource items during this portion of the assessment. When on leash for a dog interaction with another dog on leash Sparky does not approach, but instead turns his body to the side and exits. In light of this behavior we feel that placement with a rescue who can provide positive reinforcement-based behavior modification is the best option at this time. Look: 5. Dog freezes and lip curls. Food: 1. No interest. Toy 1: No interest. Rawhide: 1. No interest. Dog-dog: 2. Dog does not approach the helper dog. Turns body to side in relation to other dog, and exits.
