Medical Summary Scan negative Female, approximately 14 years old Shy, tolerated handling, tried to bite when trying to do a nail trim Cataract in right eye. Lump on left side of chest. Hair thinning on joints, and back. Fractured teeth, as well as tartar buildup and calculi buildup. Trimmed most nails until patient became too stressed, started to cough, but wasnt having difficulkty breathing. Scheduled vet check on 5/12/15 for geriatric exam. Applied Activyl Plus for flea/tick prevention NOSF BARH

Sparkles is a 14 year old female terrier mix who is being surrendered because the family can no longer properly take care of her due to circumstances at home. Upon intake she was loose for all handling including scan for microchip (negative) collar and photograph.
