S/O: Appearance: BARH; Integument: Dog is pruritic with large amount of flea dirt, some hyperpigmentation and lichenification of perineum; EENT: OU clear, AU clean, No nasal d/c; Oral cavity: MM pink, CRT <2sec, Grade 4/4 dental dz; PLN: WNL ; H/L: No murmur or arrhythmias, Lungs clear (typical brachycephalic referred airway sounds); ABD: Soft non painful; U/G: Observed dog to be peeing small amounts around exam room; prostate palpates normal on rectal exam; M/S: Amb x 4; BCS: 6/9; Neuro: NSF; NOSF
A: Severe dental dz, fleas, pollakiuria DDx: BPH, Urolithiasis, UTI, Open
P: Recommend dental prophy (will need extractions) when placed; put in medical to observe urination patterns
PROGNOSIS: Good
BASIC INFORMATION
Simon is a 12 year or male pug mixed. He was surrendered because previous owner was unable to take on the task of taking care of him. There are no known possible health problems or injuries.
SOCIALIZATION
He has no history of biting a person or animal.
BEHAVIOR
Simon uses the bathroom out on the terrace.
FOR A NEW FAMILY TO KNOW
Simon likes stuffed animals to play with and is mostly kept indoors until he has to use the bathroom. He eats both wet and dry food. In the house is well behaved.
BEHAVIOR DURING INTAKE
Simon was friendly and approached counselors. He was able to be scanned (negative) collared and photographed.
