STRAY – STRAY WAIT, NO HOLD Reason STRAY

Medical Behavior Evaluation GREEN
Medical Summary Scan negative Neutered;~ 4 years old Very friendly and sweet Cloudy left eye; squinting Seen by Dr. 1067 Administered 1/2 25mg rimadyl tablet PO Geriatric AMBx4 BARH NOSF
Weight 11.2

Sheldon is a sweet Chihuahua that was found as a stray. He is has some swelling in his eye and is uncomfortable in our care center. He will need to see an ophthalmologist in order to have his eye evaluated. Help him get to the vet!

MEDICAL:  Re-exam 05/29

S/O:  Patient presented as stray.  BAR, very uncomfortable with OS.  Constantly rubbing at it and vocalizing in pain.  Tolerable for exam.

Corneal opacity over entire cornea, OS – unable to view tapetum, No obvious signs of ulcers, Moderate periorbital swelling and erythema OS, Moderate conjunctivitis OS, Mild amount of dried serous discharge periorbitally, OS

A:  Periorbital swelling/inflammation – r/o ocular trauma vs. previous ulceration vs. glaucoma vs. other

Px:  Good

P:  No FDS uptake,  Applied TAB ointment,  Gave injection of Rimadyl (patient not interested in food) –  0.22 mL SQ,     Continue to monitor – recheck EOD

Re-exam 05/30

S/O:  Vet check to monitor appetite for rimadyl administration orally.  Patient is BAR,  Still tries to rub eye, but e-collar remains intact

Appears slightly more comfortable, Eating well – ate food with rimadyl overnight, Umbilical hernia – approx. 1-2 cm wide, reducible

A:  Periorbital swelling, corneal edema, Umbilical hernia

P:  Continue current treatment plan –   Seek New Hope Placement
