RE-EXAM: 09/15/15 09:42

S/O: QAR in cage but BAR on exam, dehydrated, underweight (BCS 2/9), severely distended and firm abdomen, female, pale mm’s, 6/6 left apical continuous heart murmur (“washing machine”), patches of alopecia, very friendly and appears comfortable/non-painful, normal RR and RE.  A: patent ductus arteriousis (PDA) – congenital heart defect causing right sided heart failure and ascites  P: new hope placement
