05/16/2015 PET PROFILE MEMO
05/16/15 16:33 Basic Info: Sandy is a 12yr old spayed female Jack Russell/Beagle Mix who was purchased at an adoption event at a petstore 12yrs ago and is being surrendered due to the owner moving to where pets aren’t allowed and personal problems. Sandy currently has a mass/tumor on her neck and reported muscle spasms when let off leash (but hasn’t occured in a year). Sandy’s last vet visit was last year for the mass and her back issues. Socialization: Sandy is friendly/outgoing, sweet & playful to all she meets. She has no experience with Cats and doesn’t like female dogs. Sandy has never bitten/scratched a person or animal. Behavior: Sandy isn’t bothered by storms/fireworks, having her food/toys/treats moved/touched, being removed off of furniture, during bathtime, having her nails trimmed, being brushed, being disturbed while resting/sleeping. Sandy will bark if someone unfamiliar approaches the home/owner. For a New Family to Know: Sandy is described as friendly, affectionate, playful, confident, very excitable, and at times independent. Her activity level is described as high and the owner loves everything about her. When the owner is home Sandy will at times follow them around or stay in her favorite spot. She enjoys squeaky toys though she’ll destroy them. She’s an indoor dog mostly but is walked 3x a day for about 30mins-1hr and pulls lightly (she can never be let off leash according to the vet due to her back problems). Sandy slept with the owner. She eats Beneful dry food 2x a day, her favorite treats are dry chicken liver from petland or the spiral tendon treats. Sandy uses wee wee pads indoors. When left alone Sandy is well behaved. Her known commands are “sit”, “stay”, “lay down”, “beg”, “cross” (when waiting to cross the street), and the owner asks her “what’s that”? and she’ll begin to sniff/hunt for things. Intake Behavior: During intake Sandy was very vocal but friendly with a loose wiggly body and opened mouth. She was seeking attention/affection from her owner and the admission counselor when we approached. She sat beautifully for pictures and allowed us to scan (neg.), collar & lift her up into a top kennel.

05/19/2015 BEHAVIOR EVALUATION – EXPERIENCE
Exam Type BEHAVIOR
Sandy walks calmly on leash. She was sociable toward the handler during the assessment – soft body, wagging tail. Sandy was calm and relaxed during handling. She was distracted, focused on objects in the room rather than the handler during the tag test. Sandy was not interested in food, toys or rawhide. Sandy was surrender to us. According to her previous owner she is friendly, outgoing, sweet & playful around strangers and children. She has no experience with Cats and doesn’t like female dogs”. The owner stated that Sandy isn’t bothered having her food/toys/treats moved or touched”. Because of her owner profile the behavior department feels that she may do best with an experienced adopter who can follow up on this behavior and can provide any necessary behavior modification. Look: 2. Dog pulls out of Assessor’s hands each time without settling during three repetitions. Sensitivity: 1. Dog stands still and accepts the touch, her eyes are averted, and her tail is in neutral position with relaxed body posture.
