Medical Behavior Evaluation BLUE
Medical Summary Sex: male intact Scan: negative S: Tense, stood still and barely moved for entire exam and vx. No signs of aggression, although pt has a history of biting. O: QAR-H, BCS 5/9, MMs pink and moist EENT: OD: pigment keratopathy, small amount of corneal opacity in the center of the cornea. No ophthalmoscope so did not perdorm ophthalmoscopic exam. OS: pigment keratopathy, cone-shaped cornea (protrusion in the center), moderate corneal opacity. No discharge AU, nose. Mild tartar on PMs and Ms. PLNs: Not enlarged. H/L: NSR, NMA. Eupnic, quiet lung sounds. Abd: Tense, no pain palpated, no masses palpated M/S/I: Amb x4. No lesions noted. UG: Male intact, testicles soft and symmetrical. A: 1. Corneal scarring and disease, possibly secondary to corneal ulcers 2. Mild dental diasease on back teeth Short-term prognosis: Excellent P: 1. Fluorescein stain, STT and ophthalmoscopic exam in near future, treat as indicated 2. Permanent vet care should include some attt\ention to dental hygience 1088
