Medical Summary 11/20/15 12:11 INITIAL EXAM microchip scan negative approx 2 year old male intact pit bull dog BAR, mild dehydration, very friendly!! mm’s pink and slightly tacky, femoral pulses S+S emaciated body condition (BCS 1/9) – severe diffuse muscle wasting, no fat on ribs or boney prominences ears clean AU eyes and nose clear, no discharge, no CIRDC signs peripheral lymph nodes WNL auscultation WNL – no murmurs/arrhythmias, lung sounds clear bilaterally adult dentition, mild tartar build up dry flaky haircoat, long overgrown nails with brown discharge at the nailbeds areas of alopecia/scarring/healing wounds over the boney prominences of the tail and on the left ischiatic tuberosity – likely pressure sores due to emaciated body condition ambulatory x 4, no lameness noted abdomen soft and non-painful, no masses palpated mentally appropriate, no neuro deficits noted male intact, two scrotal testes, smooth and symmetrical, rectal exam not performed prognosis: good PLAN: gave 2.5 ml pyrantel PO applied activyl topically start re-feeding plan due to emaciation: feed 1/2 can or cup of wet or dry Hill’s i/d TID x 7 days, then continue to feed i/d for ideal weight
