I came in the shelter as a OWNER SUR on 04/28/2015 from NY 11208, owner surrender reason stated was PERS PROB. I came in with Group/Litter #K15-011771.

05/10/2015 Exam Type RE-EXAM – Medical Rating is 3 C – MAJOR CONDITIONS , Behavior Rating is AVERAGE, Weight 41.6 LBS.
Hx: Recheck happy tail. Bandage must have fallen off because there is tape around pt’s tail but no bandage. S: Active, calm in cage, wiggly when examined O: Tail is healing well – small scab at tip only, tail not erythematous. A: 1. Happy tail healing well 2. URI Short-term prognosis: Excellent P: OK to leave tail unbandaged for now. Recheck in 3 days. 1088

DOG INFORMATION SHEET Basic Information: Rex is a 1 year old Pit-bull Terrier male who was found tied to a gate a year ago and owner had him ever since. Owner surrendered Rex because of personal problems in her life. Owner stated the two things she likes about Rex he was loveable and playful. Social Life and Personality: Rex lived with two adults and children ages 9, 11, 15 and 19 years old. Rex is relaxed, playful, and respectful towards the children in the household. Rex plays gentle with the children in the household. Rex lived with two other dogs and one being Twin A1034630 and a Pit-bull/Boxer mix. Owner stated Rex and Twin are bonded and do everything together. Rex was playful and relaxed towards the other dog in the house as well. Owner stated Rex will bark and whine towards dogs he sees outside because he wants to play. Owner stated Rex is aggressive and fearful towards cats he sees outside for walks. Rex is very house trained and goes for walks three times a day. If he’s not going for a walk he uses the bathroom outside in the yard. Rex’s activity level is very high says his owner. Owner stated Rex is playful, excited, affectionate, and friendly. Behavior: Owner stated Rex barks too much and destroys stuff indoors. Rex barks when unfamiliar people approached the house or family member. Rex tends to destroy household items such as garbage and cord if he home alone loose. Rex never bit or scratched anyone since owner had him. Rex is friendly when owner touches his food while he is eating, taking treats or toys out of his mouth. Rex is friendly with taking bath. Rex never had his nails trimmed, and can be hard to restrain if another dog he doesn’t know is around. Rex isn’t bothered by being disturbed while sleeping. For a New Family to Know: Rex knows commands such as sit, don’t move, and put it down and nigh night. Rex likes to play with balls, squeaky toys, ropes and bottles. Owner stated Rex is crate-trained. When no one is home he is kept in a crate and only barks inside of the crate if he has to use the bathroom. Rex likes to follow owner around the house or be in the same room as owner. Rex loves to sleep with people. Intake Behavior: Rex was whining because he was separated from his brother Twin A1034630 and wanted to play, his owner believed. Rex got loose during intake at one point and ran into adoptions and tried to interact with the cats. Rex was loose body and allowed all handling.

Rex came to us as an owner surrender on 4/28/15. It was reported by the previous owner that Rex has lived with children before and is relaxed, playful, and respectful towards children. Rex was also housed with another dog, and is noted to have been relaxed and playful toward the other dog in the house. Rex was very relaxed during the assessment and allowed the assessor to conduct all tactile assessment items without displaying any signs of aggression. He did not resist being handled while eating, and was easily pushed out of the food bowl. Rex showed no interest in the toy or rawhide items on this portion of the assessment. Rex is reported to be chew on some household items when left alone for long periods, so we advise future adopters to make sure to provide mentally and physically stimulating chew toys to keep Rex occupied when left alone. Look: 1. Dog holds gaze with soft eyes, soft body. He allows head to be held loosely in Assessor’s cupped hands. Dog holds gaze for three full seconds. Sensitivity: 1. Dog stands still and accepts the touch, his eyes are averted, and his tail is in neutral position with relaxed body posture. Dog’s mouth is closed for a portion of the assessment item. Tag: 1. Follows at end of leash, body soft. Squeeze 1: 1. Dog gently pulls back his paw. Squeeze 2:1. Dog gently pulls back his paw. Food: 2. Dog follows the dish with his tail down, body soft. Dog lifts head after a moderate pressure applied from hand to cheek. Toy 1: No interest. Rawhide: 1. No interest.
04/30/2015 GROUP BEHAVIOR EVALUATION – AVERAGE

When off leash with other dogs, Rex is a bit sexually motivated at first but will explore the yard. He engages in a bit of play and offers play bows. 5/2 update: Rex is playful and appropriate. He will engage if solicited and is respectful with dogs who are not interested. 5/3 Update Rex is playful but can be a bit forward when attempting to play. He does not fully engage in play and explores the yard with another dog 5/4 Update Rex offers play bows to another female who reciprocates with bouncy play bows. He continues to be social and friendly with other dogs 5/5 Update Rex is a helper dog today. He is polite and not pushy when interacting and offers appropriate corrections to an overeager puppy.
