08/07/2015 PET PROFILE MEMO
08/07/15 16:30hrs Puppy is a 7 year old neutered Chihuahua. He has been cared for by his owner for his whole life but he is being surrendered due to personal issues. SOCIAL LIFE & PERSONALITY Puppy is described as a playful and affectionate dog. He has lived with children aged 6 to 17 years old and interacted well with them. He allows petting and being picked up. He shared his home with his canine friend Mama (A1046994). They could be found playing, eating and sleeping together. Puppy will bark at other dogs he notices on the street but just until he can go near them and sniff them. BEHAVIOR Puppy is wee wee pad trained and would be walked outside only when his owner had a chance to. He is walked off leash and will stay near his owner. He will struggle to get away when he is being bathed, but tolerates being held. When he hears the doorbell ring, he will bark and continue to bark at the visitor until he is acknowledged and petted. FOR A NEW FAMILY TO KNOW Puppy is a playful dog that enjoys attention. He is well behaved when left alone with Mama in the home, however he will bark and whine if he is left alone in the house completely alone. He likes playing tug and chewing on balls and squeaky toys. He knows the commands “sit” and “come.” He does become nervous around loud sounds and will howl. He is fed Pedigree, 1 cup of dry dog food every day. He sleeps on pillows inside of his open crate at night. DURING INTAKE Puppy approached the Admissions Counselor sniffing. He allowed collaring, scanning for a microchip (positive) and taking photographs without displaying negative behavior.

08/09/2015 BEHAVIOR EVALUATION – AVERAGE
Exam Type BEHAVIOR
Puppy came to us as an owner surrender along with another small dog. It is reported the two dogs played, ate, and slept together. He has lived with children aged 6 to 17 years old and interacted well with them. Owner states Puppy will bark and whine if left in the house completely alone, but was well behaved while alone with the other dog. Puppy approached the Admissions Counselor sniffing. He allowed collaring, scanning for a microchip and taking photographs without displaying negative behavior. He walked nicely on his relief walk, no pulling. Puppy allowed his head to be help in assessor’s hands, body neutral. He stood still and accepted the touch during Sensitivity item, ears and tail neutral, body soft. Puppy followed at the end of the leash for Tag game, body soft, settling instantly at end of game, stopping as soon as assessor stopped. He did not pull his paw back on both Squeeze attempts. Puppy ceased eating and lifted his head out of the Food bowl when approached by assess-a-hand. He had no interest in Toy or Rawhide. Off leash with other dogs, Puppy greets politely but then wanders the pen. The Behavior Team feels Puppy would do well in an Average home. Look: 1. Dog’s eyes are averted. His ears are back, body neutral, and he has a relaxed body posture. Dog allows head to be held loosely in Assessor’s cupped hands. Sensitivity: 1. Dog stands still and accepts the touch, his eyes are averted, body soft, and his tail is in neutral position with relaxed body posture.
