Intake condition EXAM REQ Intake Date 06/02/2015, From NY 11691, DueOut Date 06/02/2015, I came in with Group/Litter #K15-016545.
Medical Initial: Green, scan negative, spayed female, weight: 37.4lbs,
cloudy occular lens, moderate dental tartar, clean ears, good appetite seen for soft food, clean coat, nails down to kwick, showed no signs of aggression during exam – easy to handle, BARH, NOSF
A shelter staff member writes:
Patches is a sweet and spunky senior Pittie who was surrendered because her owner was ill and could no longer care for her. Patches has an infectious energy about her, and you can’t help but smile when she starts to wiggle as you approach! She did very well on her SAFER, and according to her previous owner, she is great with kids and loves the company of cats! Patches does have a grade III heart murmur that will require some follow up care, but this petite lady has lots of life and love left to give! She really enjoys Kongs and exploring the backyard, and will make a great addition to just about any family. Help Patches find a great place to call home, and pull her from BACC today!
MEDICAL
Re-exam: 6/05
S/O: Vet check for geriatric exam. BAR on presentation. No c/s/v/d. Cooperative for exam.
mm = pink, moist
hydration = adequate
Integ = curvilinear scar on right lateral stifle. Erythematous pinnae; otherwise, good haircoat.
EENT = nuclear sclerosis, O; Mild ceruminous debris AU, nnd, unable to view dentition due to muzzle.
H/L = grade III/VI holosystolic heart murmur ausculted on the left side. Mildly harsh lung sounds. Patient was trembling.
Abdomen = snp, thin loops of intestines palpated; Rectal exam revealed moderately filled anal glands (expressed).
LN = wnl
CNS = appropriate
MS = BCS: 3/9; amb x 4; no obvious lamenesses
A: heart murmur
Px: Fair
P: Continue to monitor while in shelter
Cleaned ear
Recommend cardiology consult wherever placed.
Owner Surrender Profile
Basic information
Patches was surrendered due to her owner’s health. She is a 15 year old dog that has lived with her family since she was two. She has no reported health issues
Behavior/ socialization
Patches enjoys playing with children of all ages. Owner states that she is great with the grandchildren and very tolerant. She also enjoys the company of cats over dogs as she lived with a cat for the majority of her life. She enjoys sleeping next to them and licking them. Owner states that Patches tends to have less tolerance with female dogs (will corrective growl, snap if they get too close), but enjoys the company of male dogs more. She just sniffs the dogs and respects their space. Patches allows for her food bowl, treats, and toys to be touched while in use. She is also tolerant of baths and nail trims. She remains social when people approach the yard or family members.
For a new family to know
Patches is house broken and will go to the bathroom outside. She loves to play with balls, and soft squeak toys. She is fed 1 cup of dry food in the morning and evening and exercises by long walks. Owner states that she rarely pulls on leash

SAFER:
Patches came in as an owner surrender but there was no previous information giving, so her past behaviors in a home are unknown. During SAFER Patches came into the assessment room friendly loose and wiggly, allowed all handling items to be conducted without issue. Patches was relaxed during the tag interaction, and during the resources portion of the assessment Patches was easy to handle when engaged with the items. Patches appears to be sociable, and did not display any behaviors concerns, so the department feels she can be suitable for an “Average” adopter.
Look: 2. Dog pulls out of Assessor’s hands each time without settling during three repetitions.
Sensitivity: 1. Dog stands still and accepts the touch, her eyes are averted, and her tail is wagging with relaxed body posture. Dog’s mouth is likely closed for at least a portion of the assessment item.
Tag: 1. Follows at end of leash, body soft.
Squeeze 1 & 2: 1. Dog does not respond at all for three seconds. Her eyes are averted and her ears are relaxed.
Food: 2. Dog follows the dish with her tail down, body likely a bit stiff. Dog lifts head after a bit of pressure from hand to cheek.
Toy: 1. No interest.
Rawhide: 1. No interest.
Dog-to-dog: When off leash with other dogs, Patches greets the other dog politely, is tolerant of being sniffed and may briefly sniff the helper dog before exploring her surroundings.
Group Behavior:
