Medical Summary S/O: Appearance: BARH Integument: matted coat, overgrown nails EENT: moderate ceruminous debris AU OS: pigmentary keratitis vwith neovascularization, nuclear sclerosis OD: pigmentary keratitis with neovascularization, morgagnian to resorbing cataract with associated lens induced uveitis (aqueous flare 4+), iriditis, episcleritis and corneal bulging. Painful red eye. Mucoid discharge OD Oral cavity: MM pink, CRT <2sec, severe tartar PLN: WNL H/L: No murmur or arrhythmias, Lungs clear ABD: Soft non painful, U/G: neutered male M/S: Amb x 4, moderate hindlimb muscle atrophy BCS: 4/9 Neuro: neg menace OD, neg PLR OD, +palp OD, other CNS WNL STT normal OD IOP: 3 OD, 11 OS A: Uveitis OD- likely lens induced Poor prognosis for return of vision Good prognosis for comfort with enucleation P: Tramadol, TAB and Prednisone
