Medical Behavior Evaluation No Initial Behavior
Medical Summary –  BARH – playful, loves belly rubs, N~9 years, L eye bulging gingiva – probable cherry eye, Ears, nose, mouth clear – mild tartar and gingivitis appropriate to age.

BASIC INFORMATION:  Neyo is a white and brown male Shih Tzu who is approximately 9 1/2 yrs. old. Neyo is fixed and appears to be in good physical health. Neyo was purchased from a pet store by his previous owner and was with them for 9 1/2 years. Neyo is described as playful and smart.

Neyo is relaxed playful respectful and tolerant around children of all ages. Neyo will play with children in a gentle manner. Neyo has been socialized with both cats and dogs and is described as playful respectful tolerant and relaxed around both cats and dogs. Neyo is gentle when playing and interacting with both cats and dogs. Neyo is house trained and rarely has accidents in the house. Neyo is mostly an indoor dog who is walked on a semi regular basis. Neyo is described as a high activity dog who is playful affectionate mellow pushy aloof confident independent excitable and friendly.

Neyo is not bothered if you place your hand close to his food or if you take away his toys or treats. Neyo does not go on the furniture and can be restrained or held with minimal effort. Neyo likes all forms of grooming behaving well when being bathed or having his nails trimmed. Neyo is friendly when strangers apporach his family or property. Neyo knows the down sit and stay commands well. Neyo will go for slow walks and can’t be walked off his leash. Neyo likes balls and stuffed for toys. Neyo has not been crate trained at this time. Neyo will use wee wee pads in the house and will also use the bathroom on tress when outside. Neyo is well behaved in the house when left alone. Neyo will shows signs of anxiety and nervousnous with loud noises. Neyo likes to eat dry food 2-3 times a day. When in the house Neyo will stay with his owner and follow them around the house. Neyo will also stay in his favorite spot of the kitchen or his dog bed. Neyo will also sleep in his dog bed.

Throughout the Intake process Neyo was well behaved and well socialized with the staff here at ACC. Neyo was frienly and playful and allowed himself to be collared and handled by staff with no difficulties.
