12/08/15 14:33 Basic Information: Nala is a 6yr old spayed female Pit Bull that has no past or current medical issues that the owner is aware of. Social Life & Personality: Nala has been in the presence of children and she interacts with them in a playful gentle manner. She comes from a home with other dogs and even with dogs she doesn’t know Nala interacts with them just fine. The owner has never had a cat around for her to interact with and isn’t sure how she would react in the presence of one. Nala is an indoor only dog that is house broken. Nala is described to be friendly, playful, and affectionate with a medium activity level. Behavior Issues: Nala has never bitten a person or another animal according to the owner. Nala doesn’y have any guarding issues when it comes to food, treats, and toys. These items can be given and taken away with no issues. Nala takes well to grooming agents like recieving a bath, having her nails trimmed, and the brushing of her coat. Nala takes well to strangers and is friendly towards them. The only time she may bark or growl is when there is a fence between her and the starnger. For a New Family to Know: Nala knows a few basic commands like sit, down, and give paw. She went for slow walks on a leash and let out into the yard for daily exercise. She enjoys all sorts of dog toys and will play with just about anything. When in the home Nala is the type to follow you around seeking attention or be in the same room. When left alone in the home Nala is well behaved and this applies to her being left in the yard as well. Nala was brought up eating dry food and the brand the owner gave was the “Stop & Shop” store brand.

Nala walks calmly on leash. She was calm and relaxed during handling. Nala had a low body position during the tag test, but followed the assessor at the end of the leash. She was not interested in toys. Nala was surrender to us. Her previous owner described her as a friendly, playful, and affectionate with a medium activity level. Nala has been in the presence of children and she interacts with them in a playful gentle manner. She comes from a home with other dogs and she get along with them. According to her previous owner Nala doesn’t have any guarding issues when it comes to food, treats, and toys”. Nala did not show any concern during the behavior assessment. The behavior department feels that she can go to an Average home. Look: 2. Dog pulls out of Assessor’s hands each time without settling during three repetitions. Sensitivity: 1. Dog stands still and accepts the touch, her eyes are averted, and her tail is in neutral position with relaxed body posture. Tag: 1. Follows at end of leash, body low and a bit fearful. Squeeze 1: 1. Dog gently pulls back her paw. Squeeze 2: 1. Dog gently pulls back her paw. Toy 1: No interest. Playgroup Notes: Nala engages in brief bouncy play with a social male, she offers play bows and is tolerant of male and female dogs.

12/15/15 10:28 S/O: BAR, eating and drinking normally. superficial abrasion on dorsal nasal planum (from rubbing on cage), 3/6 left sided heart murmur, lung sounds clear bilaterally. no nasal discharge, no coughing, no CIRDC signs on exam. started on baytril yesterday. mm’s pink and slightly tacky (likely mildly dehydrated), femoral pulses S+S. temp normal on re-check last night (102 F) A: possible early/mild CIRDC abrasion on nose heart murmur overweight dehydrated P: re-check temp today LRS 750 ml SQ SID x 2 days apply SSD to nasal abrasion BID x 5 days continue baytril as previously rx no vomiting/diarrhea noted overnight or in cage this AM – CTM S: ACO noted vomiting approx 3 times- yellow frothy material @5:50PM O:T = 104 degrees Fahrenheit. BARH. Holosystolic III/VI murmur. Nasal abrasion (being currently treated). Non-painful abdomen on palpation. Dog ate one can of A/D very well, during PE. A: Nasal abrasion from rubbing on bedding/cage. “Vomited” material is probably the result of strong coughing. R/O CIRDC, mild CHF, endocarditis (though no rub ausculted) P: Baytril injectable 3.5 cc IM R. caudal gluteal mm.@6:25PM Baytril 136 mg tab: 2 PO SID starting 12/15/15 AM Cont. SSD application to nasal abrasion Rec. New Hope placement and full diagnostic workup Presented this dog’s H&P and tx to Dr. 1046 and advised of possible need for evaluation, if dog decompensates. Message left with 0811 re: transfer to MACC. Email sent to MACC overnight LVT re: transfer. PROGNOSIS: Fair to Good with treatment
