Upon intake dog was very shy. He was very shakey, but allowed all handling. Dog scanned positive for a microchip. He allowed to be collared by an admissions counselor.
Client came to drop off a dog that had belonged to his grandmother who unfortunatley passed a little over a month ago. Client stated he adopted the dog from us and gave it to his grandmother. Client stated she has been taking care of the dog for the entire 11 years. Although client had the dog for the past month, he claimed he did not know much about him. No profile was able to be created.
When off leash with other dogs, Max does not interact or attempt to play
MEDICAL:
S/O: Vet check for geriatric exam. Patient was resting comfortably on presentation. When cage door opened, stood up and gave attention. Cooperative for exam. No food present in cage, no urine/stool. QAR. No c/s/v/d reported., mm = pink, sl. tacky, CRT<2sec., hydration = adequate, Integ = Severely matted, dirty coat, fecal staining around hind fur, EENT = Mature cataracts present, OU; mildly hyperemic sclera, pigmentary keratitis present OS. Non-visual. Mild waxy debris and many hairs within ear canal, AU, Moderate/severe dental disease, H/L = nma, nsr, pss; clear lung fields bilaterally, Abdomen = snp, no masses palpated, normal rectal exam, GU = Neutered male, small bladder, LN = wnl, CNS = absent menace, no dazzle, OU; palpebrals present OU; mentally appropriate. , MS = BCS: 4/9; amb x 4,
A: Severely matted, Cataracts, Dental disease
Prognosis: Good
P: Needs clipping/grooming with bath, Continue to monitor while at BACC, Rec. to New Hope – rec. full blood work +/- ophtho consult, dental cleaning.
