MEDICAL:
8/19: S/O: BARH, clear EENT, fecal staining under tail, area of skin irritation on medial aspect of RRL. Normal auscultation. ABD SNP, NOSF. A: Applied SSD to skin on leg, needs grooming. Prognosis: Good
8/17/15: scan negative, neutered male, weight: 31.4lbs, cloudy occular lens, clean ears, clean coat,gingivitis present along with tartar and dull teeth, nails slightly long, little overweight, overall body condition is good, sweet boy – easy to handle
PROFILE:
Lucky is a 11 year old male neutered poodle mix dog who is being surrendered because the family can no longer take care of him. Lucky has been with the family for 10 years.
Lucky lived with 4 adults and 1 small child. Lucky is described to be very tolerant of the child and affectionate with the family but is also independent. Lucky is very insistent on leash when walking and gets very aroused by other dogs he sees on walks. He is house broken and does not have accidents inside.
Lucky is reported to be good at the groomers for bathing and nail trims. He will not guard his food but will flee with his bowl if he feels someone is going to take it and will do the same with toys. Lucky does not mind being handled.
Lucky enjoys stuffed animals that he likes to destroy and does not play any games with the owners. When the family is home he likes to be where they are and when alone he is well behaved. Lucky enjoys wet food. When strangers approach he will alert the family.
Upon intake Lucky was receptive to introduction and was loose for all handling. Counselor was able to scan for microchip (negative) and collar. Lucky walked well on leash and was responsive to guidance.

Lucky previously lived with 4 adults and 1 small child. The owner stated Lucky is described to be very tolerant of the child and affectionate with the family but is also independent. The owner mentioned Lucky is house broken and when home alone he is well behaved.
At intake Lucky had loose body language and allowed all handling. During SAFER Lucky came into the assessment room calm and quiet, he was friendly towards the assessor, allowed all handling items to be conducted without issue. Lucky was relaxed during the tag interaction, and during the resources portion of the assessment Lucky was easy to handle when engaged with the items. Lucky appears to be sociable, and did not display any behaviors concerns, so the department feels he can be suitable for an “Average” adopter.
Look: 1. Dog’s eyes are averted. His ears are back, his tail is down, and he has a relaxed body posture. Dog allows head to be held loosely in Assessor’s cupped hands.
Sensitivity: 1. Dog stands still and accepts the touch, his eyes are averted, and his tail is in neutral position with relaxed body posture. Dog’s mouth is likely closed for at least a portion of the assessment item.
Tag: 1. Follows at end of leash, body soft.
Squeeze 1: 1. Dog gently pulls back his paw.
Squeeze 2: 1. Dog does not respond at all for three seconds. His eyes are averted and his ears are relaxed.
Food: 1. No interest
Toy 1: No interest.
Rawhide: 1. Dog settles close, keeps a firm grip and is loose and wiggly. He does not place his body between you and the rawhide.
Dog-to-dog: When off leash with other dogs, Lucky is social and greets other dogs politely.
PLAYGROUP: When off leash with other dogs, Lucky is social and greets other dogs politely. 8/19 update: Lucky is tolerant of interaction but spends most of his time exploring his surroundings. 8/20 update: Lucky allows greeting but is more interested in exploring the yard.
