I am a female brown and gray shih tzu mix. The shelter thinks I am about 12 YEARS old.
I came in the shelter as a STRAY on 11/19/2015 from NY 11421, owner surrender reason stated was STRAY.

11/21/2015 BEHAVIOR EVALUATION – AVERAGE
Exam Type BEHAVIOR
Helper dog: a1052375 Leo came in as a stray, so his past behaviors in a home are unknown. During SAFER Leo came into the assessment calm and quiet, he was friendly towards the assessor, allowed all handling items to be conducted without issue. Leo was unresponsive but not fearful during the tag interaction, and during the resources portion of the assessment Leo was easy to handle when engaged with the items. Leo appears to be sociable, and did not display any behaviors concerns, so the department feels he can be suitable for an “Average” adopter. Look: 1. Dog’s eyes are averted. His ears are back, his tail is down, and he has a relaxed body posture. Dog allows head to be held loosely in Assessor’s cupped hands. Sensitivity: 1. Dog stands still and accepts the touch, his eyes are averted, and his tail is in neutral position with relaxed body posture. Dog’s mouth is likely closed for at least a portion of the assessment item. Tag: 2. Dog is not fearful, but is unresponsive to the Assessor, and approaches the Assessor at the end of the game (need coaxing to approach.) He is focused on stimuli other than the Assessor. Squeeze 1: 1. Dog gently pulls back his paw. Squeeze 2:1. Dog gently pulls back his paw. Toy 1: No interest. Dog-dog: 2. Dog does not approach the helper dog. Turns body to side in relation to other dog and exits.

Scan : negative Sex: female Approx 12 year S/O: QAR, depressed. Grade 4/4 dental disease. Missing most teeth. Bilateral mucoid ocular discharge, severe matting around eyes. Generalized muscle wasting BCS 3/9. Moderate skin tent. 7 cm firm, cutaneous mass, w/ pockets of palpable fluid on left side. Moderate dark debris AU, hyperpigmentation and thickening of pinna and external ear canal. Swollen vulva. Abd SNP, cardio thoracic auscultation was WNL. Appetite ++ Schimer tear test: 10mm/min OD, 8mm/min OS. Trimmed nails, shaved down matting around eyes. Gave 200ml SQ LRS A: Geriatric, poor body condition, mass, KCS, dehydrated, chronic otitis externa. Suspect underlying neoplasia. P:Recommend placement w/ rescue, notified NH Prognosis: GUARDED
