DOG INFORMATION SHEET Basic Information: Client had King for only a couple days and provided no info other than he was hyper and mouthy. Intake Behavior: Throughout the admissions process, King was very aroused; he was pulling, jumping, and was very mouthy. He allowed all handling without any aggressive behavior (no growling, snapping, or showing teeth). DOG INFORMATION SHEET (information from previous owner) Basic Information: King is a 9 month old altered pit-bull mix. He was given as a gift back in February. Owner surrenders King due to personal problems. Social Life and Personality: King lived with 2 adults and teenagers ages 13 and 14 years old. Owner stated he is playful towards everyone in the household. Owner stated he is tolerant of other dogs he sees in outside on walks. King lived with a cat and was respectful and tolerant of the cat. King is indoors and outdoors because of the backyard as well. His activity level is very high owner describes King as playful, excitable, affectionate, mellow, independent and friendly. Behavior Issues: King never bitten or scratched anyone owner stated. He is hard to control and restrain. He destroy household items such as walls, doors, furniture and baseboard for the window. He allows owner to touch his food bowl while eating but growls if anyone tries to take a toy away from his mouth. He loves to take bath and showers. He barks at unfamiliar people who approaches the house or family member depending on who it is. For a New Family to Know: King is house broken and uses the bathroom outdoors on walks twice a day. The only command he knows is sit and be quiet. He likes to play with stuffed animals, rawhides and ropes. Owner stated he whines in a crate so he doesn’t do well. He likes to follow owner around the house but when home alone destroys walls. He’s afraid of cars and traffic. He eats wet food called Pedigree and dry food called Beneful. Intake Behavior: Upon intake King was overly excited, anxious and loose body. He allowed all handling.

Helper dog: A1060369 King previously lived with 2 adults and teenagers ages 13 and 14 years old. The owner stated King is playful towards everyone in the household. The owner mentioned King lived with a cat and was respectful and tolerant of the cat. It was reported by the owner King is house broken and has a high activity level. The owner describes King as playful, excitable, affectionate, mellow, independent and friendly. At intake it was reported King was excited, anxious but maintain loose body posture, he allowed all handling. During SAFER King came into the assessment room aroused, lunging and growling at the handler, not all handling items were able to be conducted as King lunge and growled at the handler. He displayed no interest in the toy item during this portion of the assessment. When on leash for a dog interaction with another dog on leash King rushes in with his body tense and tail well above spine. In light of this behavior we feel that placement with a rescue who can provide positive reinforcement-based behavior modification is the best option at this time. Toy 1: No interest. Dog-dog: 3. Dog approaches the helper dog by rushing in with his tail high, stiff body, head tall, and ears erect.

When off leash with other dogs, King is a bit nervous but greets politely. He becomes more comfortable and attempts to solicit play and engages in play briefly with a playful female. 10/16 update: King is a bit nervous but attempts to solicit play. He will engage in brief bouts of play and may become pushy, using his paws in his attempts to solicit play.
