Medical Summary Scanned negative; QARH – patient, allows all handling; N~14 years; Cataracts completely covering both eyes. Ears dirty – cleaned with Otic.  No sign of infection; Nose, mouth clean; Teeth – heavy tartar, receding gums, halitosis. Small skin growths on base of neck, left shoulder; Coat dirty.  No flea dirt or parasites noted. NOSF

BARH; EENT: ddz G4, hair in canals AU, severe cataract OU; MS: BCS 3.5/9–slightly thin; INT: full haircoat, L sided 1 cm dermal mass; H/L WNL; GU MN; LN WNL; ABD: snp

A: 1) severe cataracts r/o secondary glaucoma, KCS 2) severe ddz 3) dermal mass r/o benign v malignant

P: recc rescue with NH for optho consult and needs dental work, FNA of dermal mass
