Medical Summary Microchip: 9851120046078 Sex: neutered male Age: reported 1y3m Mentation: dull but aware of surroundings Eyes: ocular d/c OU Ears: some hair irritation Nose: no d/c Teeth: good, gingivia irritated at insertion points If abnormal BCS: sudden weight loss reported, can easily feel ribs, anorexic for past two weeks Skin: WNL Hair Coat: WNL, some mats along ears Declawed: N/A Any injuries: no; reported health issues when visiting mobile clinic for frank blood in stool and “bloody drool”–blood work (listed as super chem, CBC, T4, with added UA; findings not presented) and suggested treatment plan suggestive of liver issues; rec from private vet to have liver shunt Sx and associated tests and hospitalization (2 view rads, glucose test 1/2/3, bile acid and resting test, ab ultrasound, k/d food, low protein diet, 0.5tab BID 10mg Pepcid AC); owner gave a history of inbreeding (this is a mother-son offspring) **paperwork inside kennel card** Behavior: friendly but dull Medication: N/A, preventatives given, held off on Vx (only req Bordetella and CIV)

NH placement ASAP – Guarded to poor Px; Seen by another vet then dropped of and surrendered; Hx: Anorexia, possible liver shunt, dull and depressed easily handled; mm- tachy and brighter pk with gingivitis and ulceration – possible start of uremia; HR 100; emaciation BCS – 1/9; GI tucked in and non painful at this time; A: Dog in poor condition Needs workup with labwork and rads; Possible kidney problem, vs liver shunt vs GI tract – FB; P: Needs placement ASAP ; LRS 75 cc BID 10 days
