03/20/2015 18:45 Dog Name: Jordan Reason for surrender: Client is surrendering dog, Jordan due to having no time to care for Jordan properly. Relationship: Client has owned Jordan for 3 months. Jordan was given to owner & has no known health issues. People: Jordan has lived with 3 adults & has been exposed to a child. Jordan behaves very well with adults and child. She is very friendly and does not turn aggressive around strangers. Jordan allows children to pet her. Other Animals: Jordan has not lived with other pets. Client stated Jordan does not like other dogs or cats. Jordan will lunge & growl at another dog or cat. Client stated when walking Jordan in the street, Jordan will growl and bark at an approaching dog. Personality/Habits: Jordan is house trained & has no accidents inside of the home. Jordan is kept mostly indoors. Jordan’s activity level is high. she loves jumping up and giving hugs, running around and playing with toys.Jordan loves to chase her own tail. The commands Jordan is familiar with are, sit, lay down, & stop. For exercise Jordan enjoys brisk walks & running around in the yard. During fire works, Jordan is not bothered. Feeding: Jordan enjoys eating both wet & dry food mixed. From the brands: Pedigree. Behavior upon intake: Jordan was very alert and friendly. Jordan did not growl or showl teeth. Jordan allowed all handling.

03/25/2015 BEHAVIOR EVALUATION – AVERAGE
Exam Type BEHAVIOR
Jordan previously lived with 3 adults and has been exposed to a child. She was said to behave very well around both adults and child, and is friendly toward stranger and other children. Jordan has not lived with other pets, and the owner stated that Jordan does not like other dogs or cats, and has been reported to growl and bark at an approaching dog when on a walk. The owner noted that Jordan is house trained has had no accidents inside of the home. Her activity was said to be high, and she loves to jump up to giving hugs, running around and playing with toys. At intake it was noted that Jordan was very alert and friendly, and allowed all handling. During SAFER Jordan was very sociable towards the assessor, and allowed the assessor to conduct all handling items without issue. She did become distracted during the tag interaction, but approached the assessor at the end of the game. Jordan was a bit stiff during the food item, but after a bit of pressure was applied to her cheek; she lifted her head from the food bowl. Jordan had a loose body when engaging with the toy and rawhide items, and pushed the toy onto the assessor with a loose and bouncy body to initiate a game of tug. Look: 2. Dog pulls out of Assessor’s hands each time without settling during three repetitions. Sensitivity: 1. Dog stands still and accepts the touch, her eyes are averted, and her tail is in neutral position with relaxed body posture. Dog’s mouth is open for at least a portion of the assessment item. Tag: 2. Dog is not fearful, but is unresponsive to the Assessor, and approaches the Assessor at the end of the game (needed coaxing to approach.) She is focused on stimuli other than the Assessor. Squeeze 1 & 2: 1. Dog does not respond at all for three seconds. Her eyes are averted and her ears are back. Food: 2. Dog follows the dish with her tail down, body a bit stiff. Dog lifts head after a bit of pressure from hand to cheek. Toy: 1. Dog settles close, keeps a firm grip and is loose and wiggly. She does not place her body between you and the toy. Rawhide: 1. Dog settles close, keeps a firm grip and is loose and wiggly. She does not place her body between you and the rawhide. Dog-dog: When off leash with other dogs, Jordan is tolerant, playful and bouncy and likes to play rough and rowdy.
