10/19/15 16:13 DOG INFORMATION SHEET Where did this dog come from? My Home Original Pet Source: Jerry came from a Pet store Does this dog have a microchip? Yes WHY ARE YOU BRINGING THIS DOG IN? Owner stated the dog was getting harder to handle and was being aggressive with everyone in the household. RELATIONSHIP Owner has had the dog for 5 years. PEOPLE Jerry lived with 2 adults and 2 young adults. Does he get along with children? No. He will not play with children and will sometimes bark. Does he get along with strangers? No Has he ever bitten anyone? Yes. Bite happened over 2 years ago. Has he bitten anyone in the last 10 days? No Describe his personality: Owner described Jerry as playful, excitable, affectionate, and anxious. Jerry is house trained and rarely has had accidents. He has a medium activity level. Jerry will growl if you touch his food, treats, take his toys away or push him off the furniture. He is friendly when being given a bath. Jerry is not bothered when someone trims his nails, brushes his coat or restrain him. When unfamiliar people approach the house, owner stated he can bite. Jerry likes to stay in his favorite spot when in the household and will bark or whine when left home alone. When in the backyard, he is well behaved. When Jerry is off the leash, owner stated he will usually be by their side. He gets nervous when there is a storm or if there are fireworks going off. Jerry has been crate trained and usually stays in one fine for a few hours each day. He loves balls and squeaky toys. OTHER ANIMALS Jerry is aggressive with other dogs and cats. HABITS Jerry was fed dry food. TRAINING Jerry knows how to sit, down, shake/give paw, and stay Does this dog have any behavioral problems that you’re aware of? If yes, please provide as much detail as possible. Owner stated that Jerry has been getting more aggressive lately and growling,barking and trying to attack everyone in the household.

Jerry came into our center as an owner surrender on 10/19/2015 citing he has started growling and barking at family members. It is reported he lived with adults and teenagers. Owner reports Jerry will guard his food and family, unpredictable behavior around strangers. Owner states Jerry will bark and growl at dogs, and does not get along with cats. With people Jerry is comfortable with, it is reported he loves to play and is very active. He is reportedly crate trained and housebroken. Owner reports Jerry pulls very hard on leash. In our center, Jerry has been very reactive while in his kennel, growling and snapping towards other dogs and people walking by. Off leash in the yard, Jerry growled at staff while being corrected. Due to staff being uncomfortable with handling, the tactile portion of the SAFER assessment was skipped. Jerry calmly allowed the Food bowl to be moved away, and required a bit of pressure to lift his head out of the Food bowl. He took Toy away, body not stiff. Jerry took Rawhide away, body a bit stiff. After assessment while in kennel, Jerry did take treats gently from assessor’s hand, body not stiff. Jerry must be placed with a New Hope partner that can monitor his behaviors and be prepared to modify any concerning behaviors that may manifest in the home. Food: 2. Dog follows the dish with his tail down. Dog lifts head after a bit of pressure from hand to cheek. Toy: 2. Dog takes toy away, keeps a firm hold. His body is between you and the toy, body neutral. No growling or stiffness. Rawhide: 2. Dog takes rawhide away, keeps a firm hold. His body is a bit stiff. dog-dog: When off leash with other dogs, Jerry was muzzled as per his profile, he appeared okay with female dog although he was quite preoccupied with the muzzle. He explored the yard without a muzzle and with the company of a female dog. When corrected by human handler to leave an area, Jerry escalated to human correction and began to growl at the handler. This happened again two more times uncorrected and unprovoked. Jerry was removed from the yard without any further introductions.
