Medical Summary 11/19/15 17:04 INITIAL EXAM microchip scan negative approx 7 year old male intact shih tzu dog BAR, hydrated, BCS 6/9 heart and lungs WNL – no murmur ausculted, lungs clear bilaterally no fleas or flea dirt seen ears dirty AU, large amount of brown discharge in left ear – otitis externa nuclear sclerosis OU, mucoid ocular discharge OU nose WNL, no discharge, no congestion or CIRDC signs integ: severely matted haircoat with urine and fecal matter incorporated in the mats, dry flaky skin under matted regions and mild dermatitis in some regions. very painful when mat on tail was shaved – tail is kinked, suspect tail fracture (will likely need a tail amputation at time of neuter) mentally appropriate, no neuro deficits noted abdomen soft and non-painful, no masses palpated ambulatory x 4 missing many teeth, severe dental disease two scrotal testes long nails PLAN: gave rabies vaccine SQ over right hind leg gave CIV SQ over left hind leg gave bordetella vaccine IN gave DA2PP vaccine SQ over right front leg gave 2 ml pyrantel PO applied activyl topically shaved as much of the matted hair as possible without sedation – he was very tolerant at first (such a sweet dog!!!) but after about an hour he started to lose patience and started growling/snapping — plan to sedate tonight at MACC to finish shaving matted hair and do a thorough ear cleaning and nail trim
