Medical Summary 12/03/15 13:19 scan neg MI 10y QAR, very weak G4 dental disease OU cataracts Integ: severe matting and ingrown nails x4 feet. foul smelling skin, anus blocked with matts, very erythematous and swollen perianal area . feces embedded in fur BCS: 5/9, very weakly ambulatory H/L: G2 L systoloic murmur, exaggerated sinus arrhythmia. crackles on ausculation. coughing GU swollen testicles, painful ABD: swollen abdomen, tense on palpation A: coughing and crackles r/o CHF v infectious v inflammatory; urine scald P: lasix 2 mg/kg IM, 0.25 cc penG. Needs x-rays and echo, grooming, antibiotics for skin infection and dental disease. recc NH placement v EHR. concern for cruelty . 0.15 cc butorphanol IM for sedation. applied epiotic to perianal area, washed with malaseb prognosis gaurded
Jake is a geriatric male Yorkie. His family got him from a pet store 10 years ago. They surrendered him because they no longer have time to care for him. Jake is heavily matted on his limbs and rear end and his matts are full of urine and feces. Much of his skin is raw. He has a strong odor. He seems to have difficulty breathing and didn’t stand up during his intake. His owner reported that he has been active/normal except that he didn’t eat his breakfast this morning.
Jake is friendly/outgoing with strangers. He lived with small children and was relaxed, playful, and tolerant with them. He met dogs outside and was relaxed, tolerant, and played gently with them. Jake has never bit anyone.
Jake is nervous during storms. He enjoys being brushed and is friendly with strangers who approach his home/family. He doesn’t mind when someone takes his food/treat/toy, moves him off furniture, holds/restrains him, or wakes him up. He’s nervous during baths.
FOR A NEW FAMILY TO KNOW
Jake’s family describes him as friendly, affectionate, mellow, and medium energy. He lived inside and slept in his owner’s room. He was trained to potty on the bathroom floor. He also pottied on grass when walked. He’s well-behaved while home alone. He knows sit. He went for slow walks and doesn’t pull on his leash. Off leash, he stays close to his owner.
Jake had a soft body and didn’t respond much while we picked him up, scanned him (-), collared him, positioned him for pictures, and put him in a kennel.
