11/25/15 20:20 This dog was very nervious during intake. He was hard body and growling. He urinated and deficated in the office. He would not allow me to touch him for more than 2 seconds. He would not allow me to collar him.

12/01/2015 BEHAVIOR EVALUATION – NH ONLY
Exam Type BEHAVIOR
Jacob was difficult to remove from his kennel showing teeth, growling and lunges at the rope. He did not show any social behavior. Once he was giving a low growl and exposes his front teeth, grabbing at the leash and preparing to lunge forward to the handler. The behavior team decided to place him back in his kennel for the safety of the dog and the handler. Due to the behavior demonstrated in the shelter the behavior department believes that Jacob is not an appropriate candidate for SAFER. The behavior department feels that he would do best if placed with a New Hope partner who can follow up on this behavior in a more stable environment and can provide any necessary behavior modification before placement into a permanent home.

11/25/2015 INITIAL PHYSICAL EXAM
Medical rating was 3 NC – MAJOR CONDITIONS NOT CONTAGIOUS, behavior rating was NONE
Intact male Scan negative for a microchip 2 years old according to the owner Eyes are clear Nose= No discharge Ears are clean. Right ear has a mat Teeth are white and intact Right side of the neck has a wound. Appears to be painful in that area and will snap. Sedation recommended for complete exam of the area. Area cleaned with Novalson solution. Coat is clean Dewormed with Pyrantel Flea Treatment= Activyl As of now no signs of CIRDC As of now no V/S/C/D NOSF Is on a vet check Does not mind being picked up with a shirt or towel wrapped around him. Will give a hard time coming out of the cage because of the leash going around his neck due to having a wound. Will snap because under his neck is painful Harness only but will dart out of the cage.
