Medical Summary S: Calm and easily handled with no signs of aggression O: Scan: negative Sex: female intact Appearance: QAR, ~5% dehydration Integument: skin severely thickend and erythmatous with lesions on the head/face/ears from scratching, no active pustules, generalized yeast smell EENT: OU mucoid discharge and severe entropion on both upper and lower lids, maloderous aural debris AU, No nasal d/c Oral cavity: MM pink, CRT <2sec, mild dental tartar PLN: WNL H/L: grade II/VI murmur, respiratory sinus arrhythmia, Lungs clear ABD: Soft non painful, no palpable masses or organomegaly U/G: M/S: Amb x 4, no appreciable lameness Neuro: NSF Skin scrape-negative for demodex or scabies Ear cytology- positive for yeast and bacteria A: Generalized dermatitis- r/o severe allergies (environmental vs. dietary) vs. other Heart murmur- r/o CVD vs. other Bilateral otitis externar/o secondary to allergies P: NO antibiotics started due to no evidence of active infection (no pustules), recommend food allergy trial +/- deratology consultation following placement, begin weekly ear cleanings and daily otomax Prognosis: good with aproprite treatment, poor quality of life without dermatologic intervention
