12/11/15 19:26 BASIC INFORMATION Hazel is a 7 month old female American Pit-bull Terrier, who was given to her owner since a pup as a gift. She is being surrendered due to her owner living situations and is unable to continue car for her. Since her owner no longer had a permanent place to live, Hazel hasn’t been eating. SOCIALIZATION Around strangers Hazel is friendly and playful, with adults she is gentle and exuberant. She lived and spent time with children that age ranged from 4month-13 years old. She’s playful, gentle and respectful when she plays with the kids. She has been around another dog which was a yoke while the Yorkie didn’t acknowledge her, but Hazel was curious and barking around the dog. BEHAVIORAL Hazel tends to poop and pee inside of the house, even after being walked for an hour as described by her owner. Sometimes when left alone she destroys things in the house such as shoes, and sometimes the walls. Due to changes in environment she started to eat her poop. She isn’t bothered when her food bowl is being touched or something has been taken away from her. When being giving a bath isn’t bothered or having her coat brushed. When strangers approach the yard she isn’t bothered or show any aggressive action or behavior. She never had her nails trimmed nor given rawhides. FOR A NEW FAMILY TO KNOW Hazel is described as playful, affectionate, and excitable, though she is shy at first when warming up to people. Her activity level is high and she likes to run around and play chase. Two things that was unique about her us her playfulness and affection when people was around she lays under them. Her favorite toys were balls and liked to play fetch with it. She kept moth indoors and out. She eats dry food and any type of treats are her favorite. Hazel isn’t house trained but she is still in the learning stages, her accidents happen daily. When left alone she destroys some things in the house. she knows such commands as “sit” but is willing and eager to learn new things. She enjoys running and jogging for exercise, also playing in the yard. When off leash he runs away and sometimes come when called. He uses the bathroom on the cement or grass. BEHAVIOR DURING INTAKE Hazel was curious and playful. Allowed all handling with counselors to be scanned (negative), collared, and photographed.

12/15/2015 BEHAVIOR EVALUATION – EXPERIENCE

Hazel came to us as an owner surrender on 12/11/15. It was reported by the owner that Hazel is playful, gentle and respectful when playing with children. Hazel is reported to show destructive behavior in the home, such as chewing shoes, and sometimes the walls. We are unaware of what, if any, attempts were made to remedy this behavior but we suggest to future adopters that Hazel will need to be provided with physical and mental stimulation and with a variety of appropriate and engaging chew toys so Hazel can engage in necessary chewing behavior without damage to human objects. She was very interested in interacting with assessor when entering the behavior room. She allowed the assessor to conduct all tactile assessment items while maintaining very soft body language. She showed no interest in the toy on this portion of the assessment. The behavior department believes that Hazel would do best with and experienced adopter that can provide any behavior modification necessary. Look: 1. Dog holds gaze with soft eyes, soft body. She allows head to be held loosely in Assessor’s cupped hands. Dog holds gaze for three full seconds. Sensitivity: 1. Dog stands still and accepts the touch, her eyes are averted, and her tail is in neutral position with relaxed body posture. Dog’s mouth is likely closed for at least a portion of the assessment item. Tag: 1. Dog assumes play position and joins the game. Or dog indicates play with huffing, soft ‘popping’ of the body, etc. Dog might jump on Assessor once play begins. Squeeze 1: 1. Dog gently pulls back her paw. Squeeze 2: 1. Dog gently pulls back her paw. Toy: 1. No interest.
12/14/2015 GROUP BEHAVIOR EVALUATION

When off leash with other dogs, Hazel greets the helper dog appropriately with a low body and head. She appears to be quite nervous when interacting with the other dog. She displays a submissive grin several times to both handler and dog. 12/17 update: Hazel greets the other dog briefly but is more interested in interacting with the handlers. 12/18 update: Hazel is nervous but will greet the other dogs briefly. She becomes overwhelmed easily but will engage in brief bouts of gentle play with some dogs. 12/19 update: Hazel is nervous but may greet the other dog briefly. She is tolerant of solicitation but is mostly focused on interacting with the handlers. 12/22 update: Hazel greets politely and engages briefly in gentle play when solicited. She may attempt to offer correction when the other dog becomes too exuberant.
