04/04/15 16:41 NAME: Hazel REASON FOR SURRENDER The nolonger has the proper amount of time to devout to Hazel and feels that she deserves better. RELATIONSHIP Hazel is estimated to be around 2. She has been with her owner for the last year. PEOPLE Hazel lived with an adult and children under 10. The owner states she is always friendly with everyone and allows people of all ages to handle her. OTHER ANIMALS On walks, the owner states that Hazel will bark and try to rush to any cat she sees, but Hazel is friendly with other dogs. She does go to the dog park for play time and greets other dogs by sniffing first. In the lobby she got loose from the owner’s hands and Hazel ran up to greet another dog (A1032155- Jeter). She had a soft body and a tail wag upon the greeting. She started to whine when she was separtated. PERSONALITY/HABITS Hazel is a 2 year old dog inside dog with a very high activity level. She likes the company of other dogs according to the owner. She is described as very friendly, playful, confident, and affectionate. She isn’t bothered if you touch her food bowl while she is eating ot take her bones or toys away from her. She is well behaved when left unattended and will follow her family around the home. The owner staes lound sounds like storms and fireworks do not bother Hazel. She is taken out twice a day and she enjoys her time outside with her people. She plays with balls and squeaky toys. TRAINING Hazel knows sit, down, come, and stay. It is very rare for her to have a potty accident if she is taken out several times a day. MEDICAL There are no known medical issues at the time of intake. BEHAVIOR DURING INTAKE Hazel was very energetic and would not be still. She had a soft body and allowed the counselor to greet her and pet her. Hazel was very easy to collar, scan, and photograph. She did meet a dog in the lobby (A1032155- Jeter) andmaintained a sloose body and a tail upon the greeting and wagged her tail. She started to whine when she was separtated.

Hazel previously lived with an adult and children under the age of 10, and the owner states she is always friendly with everyone and allows people of all ages to handle her. Hazel was reported to bark and try to rush to any cat she sees, but Hazel is friendly with other dogs. She was said to have a very high activity level, and was described as very friendly, playful, confident, and affectionate. It was also stated that Hazel is well behaved when left unattended, and it is very rare for her to have a potty accident in the home. At intake Hazel was reported to be very energetic and was in constant movement, but allowed all handling and was easily collard. During SAFER Hazel was very sociable towards the assessor, and had a relaxed body posture throughout the assessment. She was playful during the tag interaction and was easy to handle when engaged with the resource items, and did not show any concerning behaviors when the items were removed, so the behavior department feels Hazel is suitable for an Average experienced adopter. Look: 1. Dog’s eyes are averted, with tail wagging and ears back. She allows head to be held loosely in Assessor’s cupped hands. Sensitivity: 1. Dog leans into the Assessor, eyes soft, soft and loose body, open mouth. Tag: 1. Dog assumes play position and joins the game. Squeeze 1 & 2: 1. Dog does not respond at all for three seconds. Her eyes are averted and her ears are back. Food: 1. Dog calmly allows the food to be moved, follows the dish, but does not interfere with the dish’s movement. Dog’s body is soft and loose, eyes soft, tail neutral. She lifts her head when hand is pushed against her cheek. Toy: 1. Minimal interest in toy, dog smells, then turns away. Rawhide: 1. Dog settles close, keeps a firm grip and is loose and wiggly. She does not place her body between you and the rawhide. When off leash with other dogs, Hazel is polite when greeting at the gate, but is a bit stiff with a wagging tail when the male helper dog. He is a bit forward in his greeting and Hazel corrects. Hazel engages in play with a tolerant female and will vocalize with sharp barks during play. 4/6 Update Hazel is eager to play and is very bouncy and will solicit play from other dogs. When play gets energetic, Hazel will offer corrections but the severity of the correction does not match the situation. 4/7 Update Engages in soft play with a male. She does not offer any mixed signals. 4/8 Update Hazel is status seeking with two male dogs today. She will engage in play but then will jump on the backs of the other dog. 4/9 Update Hazel engages in a bit of play and offers play bows. 4/10 Update Hazel plays well with a younger dog 4/12 Update Hazel enjoys a bit of rough and rowdy play with a friendly female.
