Upon intake Frisbee was quiet but receptive to introduction and loose for all handling. Counselor was able to scan for microchip (negative) collar and photograph.

PE: EENT MATURE CATARACTS OU, PLN WNL, ORAL EXAM SEVERE DENTAL DISEASE, THORACIC AUSC WNL, ABD PALP WNL, M/S WNL, SKIN/HAIR — SEVERE MATTED AREAS OF EXTREMITIES — UNKEMPT HAIRCOAT. NUMEROUS WART-LIKE MASSES ON SKIN, NAILS ARE EXTREMELY OVERGROWN — STRANGE THICKENNED LATERAL LEFT FORELIMB NAILBED (IRREGULARLY KERATINIZED), INTACT MALE (SOFT/SMALL TESTICLES) , PROVIDED 0.15ML TELAZOL IM AND MAINTAINED ON ISOFLURANE FOR COMPLETE SHAVE/CLIP DOWN , PROVIDED 0.1ML BUPRENEX SR SQ AND 0.8ML CONVENIA SQ, GERIATRIC SICK, PROGNOSIS IS GAURDED TO FAIR — RECOMMEND FULL GERIATRIC WORKUP WITH LOCAL VETERINARIAN WITH PLACEMENT.
