Medical Summary scan: 440A525237 sex: intact male .5 activyl applied .9 dewormer given S/O: Dog is QAR, BCS 1-2/9 (very thin), eyes and nose wnl, AD – large amount of mucopurulent discharge, appears to have aural mass in canal, AS – wnl, grade 1-2/4 dental dz, grade II/VI heart murmur, lungs auscult wnl, abdomen soft and non-painful, intact male – both testicles descended A: Aural mass and severe otitis externa AD; mild heart murmur; underweight P: Gave DA2PP, BB, CIV, and Rabies; gave 0.9cc Pyrantel PO, 0.5cc Activyl applied; cleaned AD; start on Baytril and Meloxicam; put in medical to monitor ear and feed TID
