I came in the shelter as a OWNER SUR on 06/20/2015 from NY 11365, owner surrender reason stated was COST.

06/22/15 10:02 PE unchanged from 6/21 A MGT P Recc surgical resection and OHE S/O: Vet check for geriatric exam, tumor, otitis. Patient is QAR on presentation. TImid for exam but cooperative and tolerable of handling. Frequently rubs right side of face on floor. No c/s/v/d. Per staff, patient has good appetite. Patient was muzzled for exam. mm = pink, moist, CRT<2sec. hydration = adequate Integ = Large, firm mass on ventral thorax, associated with R1 mammary gland, seems to extend caudally from it. Approx. 5cm x 3 cm, SQ. Appears to have deformity in distal aspect of tail; abnormally bent – likely old fracture EENT = early nuclear sclerosis OU, clean AS; malodorous AD with tan liquid debris, mildly erythematous – patient resents examination of ear. Attempted otoscopic exam – canals are sensitive and oozed mild blood. Unable to view TM due to patient’s discomfort. Unable to perform full oral exam due to muzzle. Abdomen = snp, nmp; rectal exam not performed GU = female, no spay scar detected. CNS = appropriate LN = wnl MS = BCS: 5/9; amb x 4 A: Geriatric MGT Otitis externa Px: Fair to good in the short term – rec. work up/removal of MGT P: Cleaned ear out – patient very sensitive – ear cleanser+Otibiotic Gave 75 mg RImadyl in pill pocket – continue PO BID x 5 days – recheck at end of ear treatment

Dynasty came to our center as an owner surrender. It is reported she has lived with adults and seniors, gets along with children and strangers. Dynasty was cautious during Look item, eyes averted. She accepted the touch during Sensitivity, giving whale eye with her tail down. Dynasty followed at the end of the leash for Tag, body soft. She did not pull her paw back for both Squeeze attempts. Dynasty had no interest in Food, Toy, or Rawhide. Dynasty approached the helper dog at an appropriate pace, ears forward and tail at spine level. The Behavior Team feels Dynasty would do well in an Average home that understands she may need time to warm up to her new environment. Since she lived with seniors, she may do best with a calm, quiet atmosphere. Look: 2. Dog’s eyes are averted. Her body posture is cautious; her tail is low and not moving. She allows head to be held loosely in Assessor’s cupped hands. Sensitivity: 2. Dog stands still and accepts the touch, her eyes are averted, her tail is between her legs, body neutral, mouth closed, lip long, ears back. Tag: 1. Follows at end of leash, body soft. Squeeze 1 / 2: 1. Dog does not respond at all for three seconds. Her eyes are averted and her ears are relaxed. Food. No interest. Toy 1. No interest. Rawhide 1. No interest. Dog – dog 2. Dog approaches helper dog with tail at spine level, body not stiff, ears relaxed, and lip neutral.
