Medical Summary Microchip: negative Sex: intact female Age: appx 12y Mentation: BARH Eyes: opacity to eyes Ears: clean Nose: no d/c Teeth: ddz (heavy calculus and odorous)–discoloration to tongue and buccal area (breed vs other) If abnormal BCS: underweight Skin: WNL Hair Coat: unkempt, many mats, brushed some out and shaved others–will need “touch up” groom Declawed: N/A Any injuries: no; appears deaf (does not respond to loud sounds, commands, etc); slow gait, possibly arthritic, no “sway:” to walk in hips Behavior: allows all handling Medication: N/A, preventatives given

A volunteer writes: It’s easy to imagine the beauty and majesty of Daphne when she was young. She’s a senior now, her coat is frayed and needs a grooming, she’s missing some teeth, her eyes are cloudy, she may be deaf, but she’s the most beautiful dog on the planet. She carries herself with dignity, steps out of her kennel gingerly and we head outside. Daphne goes potty immediately and climbs the steps up into the yard slowly. She may have some age related arthritis in her back legs but she’s a trooper and she’s not going to let a little arthritis get her down. Off leash she takes treats gently, is a bit hand shy, but when she trusted me she allowed me to pet her leaning into my hand. As I pet Daphne I can feel her spine as she’s emaciated and needs to gain some weight. Daphne wags her tail at dogs in adjoining pens, perhaps remembering what it was like to be young and frolic with doggy friends. The more time I spent with Daphne the more beautiful she became. She’s looking for her retirement home, a soft bed, nutritious food and hands to love her. She’ll win your heart as soon as you meet her just as she won mine.
RE-EXAM 12/26/15
S/O: BAR, hydrated, reportedly possibly deaf (difficult to confirm on physical exam, but did not appear to respond to auditory stimuli), wound on right side of face (under right eye) with large amount of dried purulent discharge – cleaned wound with nolvasan, appears to be healing (has granulation tissue formation). right hind leg (proximal part/thight) is edematous/swollen but non-painful on palpation. BCS 2-3/9 with boney spine/muscle wasting. alopecia and flaky skin on dorsum, nuclear sclerosis OU, moderate dental disease, heart and lungs WNL; A: geriatric; wound; possibly deaf; underweight; alopecia; swelling in right hind leg; P: start clavamox 250 mg PO BID x 10 days; recommend new hope placement for geriatric work up
