11/15/15 10:49 BASIC INFO Cuquita a1057962 is an aprroximately 9 year old Maltese Mix. She was a gift to the Owner from a friend, 6 years ago. the main reason for surrender is that the Owner is moving to a place where pets are not allowed. Cuquita does not have any health issues or concerns. SOCIALIZATION Around strangers, she is shy for a few minutes then warms up. Cuquita has lived with a 9 month old Baby and she was bvery affectionate, and respectful of the child. she plays gently with adult and loves to play with a squeaky toy. she has not lived with any other animal. Cuquita has never bitten or anyone. BEHAVIOR Cuquita does not have any behavior issues. During fireworks or loud noises she is afraid. Cuquita is not bothered if you take away her food bowl, water bowl, toy, or anything away. She allows you to bathe her and enjoys being pet. If an unfamiliar approaches the door, she barks. FOR A NEW FAMILY TO KINOW Cuquita is best described as friendly, affectionate and playful. she has a low activity level as she is a geriatric dog. At home, she follows you around. cuquita has been kept mostly indoors. she sleeps on a pillow or by the door on the floor. she eats Beneful dry dog food, and goees p[otty on wee wee pads. when left home alone, she is well behaved. For exercise she would be slow walked on the leash. BEHAVIOR DURING INTAKE Cuquita was a little nervous during intake and was focused on her owner while we took pictures. She allowed all handling, collaring and photos.

11/17/2015 BEHAVIOR EVALUATION – AVERAGE
Exam Type BEHAVIOR
Cuquita walks calmly on leash. She was sociable toward the handler during the assessment – soft body, low wag. Cuquita was calm and relaxed during handling. She was distracted, focused on objects in the room rather than the handler during the tag test. Cuquita was not interested in toys. Cuquita was surrender to us. According to her previous owner Cuquita around strangers, she is shy for a few minutes then warms up. Cuquita has lived with a 9 month old Baby and she was bvery affectionate, and respectful of the child. She plays gently with adult and loves to play with a squeaky toy. She has not lived with any other animal. Cuquita is not bothered if you take away her food bowl, water bowl, toy, or anything away”. Cuquita did not show any concern during the behavior assessment. The behavior department feels that she can go to an Average home. Look: 1. Dog’s eyes are averted. Her ears are back, her tail is down, and she has a relaxed body posture. Dog allows head to be held loosely in Assessor’s cupped hands. Sensitivity: 1. Dog stands still and accepts the touch, her eyes are averted, and her tail is in neutral position with relaxed body posture. Tag: 2. Dog is not fearful, but is unresponsive to the Assessor, and approaches the Assessor at the end of the game (may need coaxing to approach.) she is focused on stimuli other than the Assessor. Squeeze 1: 1. Dog does not respond at all for three seconds. Squeeze 2: 1. Dog does not respond at all for three seconds. Toy 1: No interest.
