Medical Summary 10/22/15 19:06 initial exam microchip scan negative approx 2 year old (owner reported) male intact mixed breed dog due to temperment he was muzzled for exam – oral cavity not examined BAR, hydrated mentally appropriate, no neuro deficits noted BCS 3/9, amb x 4, no lameness, mild carpal valgus but walks normally full haircoat with matting around ears and a few minor mats on dorsum — shaved during exam skin WNL, no parasites seen no ocular or nasal discharge, no coughing, no CIRDC signs male, intact, two descended testicles smooth and symmetrical heart and lungs WNL on auscultation clean ears, long nails NOSF PROGNOSIS: excellent PLAN: gave CIV vaccine SQ over left hind leg gave rabies vaccine SQ over right hind leg gave DA2PP vaccine SQ over right front leg gave bordetella vaccine intranasally gave ml pyrantel PO applied activyl topically
