03/27/2015 Exam Type CAGE EXAM – Medical Rating is 3 C – MAJOR CONDITIONS , Behavior Rating is NONE, Weight 58.1 LBS.
3/27 coughing today keep on current meds but move to iso recheck 3/29 (last day of baytril) 3/25/15 removed e-coller, no obvious pruritis some yellow discharge around eyes. will add TAB and clavamox for skin fair prognosis (good with proper treatment) 03/23/15 19:22 update: initial skin slide negative, on for night tech scraping with handler malezzia suspected, begin ketoconazole bath with malasseb shampoo also indicated, setting up on tech check —————- alopecia, reportedly infested with fleas hair loss on ventrum, legs, face ddx distribution of other ectoparasites adding baytril for secondary pyoderma recheck in 7 days, poss skin scraping spayed, defect in linea consistient with sutures ———————- BARH scanned negative no ear mites but dry scabbing skin -croped ears flea tx given of activyl mild dental staining Female allopecia vs flea dermatitis vs other -will monitor with vet for tx bone deformaty due to breed – wide broad low stand -limbs curve very friendly wagging pale seeking attention and well behaved

03/22/15 13:38 Chloe was found wandering loose in the street. She was brought here by police. They report that she was very sweet, friendly, quiet, and well-behaved with them. The skin on Chloe’s legs, tail, face, underside, and rear end are pink with lots of scabs and some open areas. Her nails are long. The skin on her ears and in spots on her head and body is hairless and grayish. Her lower abdomen and rear end have loose skin and her nipples are very large, as if she has had puppies. Chloe had soft body and eyes during all of intake. Her tail wagged fast when I approached, talked to, and handled her-scan (negative), collar, switching leashes, petting, pictures. She rested her face gently in my hands and looked up at me. She kicked her back feet a little while I scratched her back. She sat next to me quietly while she waited.

A volunteer writes: It is obvious why Chloe was found wandering the streets of New York…What I can not understand is why Chloe’s owners let such a wonderful dog get so run down..She gave them puppies and more than one time….She was a “good girl” who did not ask for much. And here she is now with us, sweet, gentle and loving Chloe who so much craves for attention. Chloe is peaceful in her kennel but comes right away to her door when she sees a human. Her tail is wagging, her body is wiggling, her naked chestnut eyes are all on me. A friend cleans her face for picture taking. She does not mind. Her nails are long and curved. Her coat is mangy. Chloe is a beauty, though. In and out. Yes, that is what she will be once she lands in your hands, well fed, well housed, respected and mostly loved. A flawless silver dress will wrap her body and she will be the talk of the town…Chloe walks well on the leash despite her Elizabethan collar. She “makes do” because she is such an easy going female. Attention is what is important to her and she rewards me profusely with kisses. Other dogs passing by do not seem to be an issue for Chloe. Approaching people and staff are most welcome. She comes when called, sits on command and takes treats gently from my hand. What is not to love about Chloe? She is our little frog waiting for the magic kiss, yours…to unveil the beautiful princess hiding under the rags. Chloe is at the Manhattan Care Center.
03/25/2015 BEHAVIOR EVALUATION – AVERAGE

Chloe walks calmly on leash. She was sociable toward the handler during the assessment – soft body, wagging tail. She was calm and relaxed during handling. Chloe was calm and relaxed when playing tag with assessor. She did not resist being handled while eating, and was easily pushed out of the food bowl. Chloe was not interested in toys but took the rawhide away but had soft body language. She approaches the helper dog with her tail at spine level; lip long, body not stiff. Chloe did not show any concern during the behavior assessment. The behavior department feels that she can go to an Average home. Look: 2. Dog pulls out of Assessor’s hands each time without settling during three repetitions. Sensitivity: 1. Dog stands still and accepts the touch, her eyes are averted, and her tail is in neutral position with relaxed body posture. Dog’s mouth is likely closed for at least a portion of the assessment item. Tag: 1. Follows at end of leash, body soft, wagging tail, open mouth. Squeeze 1: 1. Dog gently pulls back her paw. Squeeze 2: 1. Dog gently pulls back her paw. Food 1. Dog lifts head and ceases eating when you reach to pull the bowl away Toy 1. No interest. Rawhide 1. Dog settles close, keeps a firm grip and is loose and wiggly. she does not place her body between you and the rawhide. Dog – dog 2. Dog approaches helper dog with tail at spine level, body not stiff, ears relaxed, and lip long or neutral. Helper:A1029429
