04/15/2015 Exam Type RE-EXAM – Medical Rating is 2 NC – MINOR CONDITIONS NOT CONTAGIOUS, Behavior Rating is NONE, Weight 38.7 LBS.
S/O: Vet check for stenotic canals. Patient appears BAR on presentation. Allowed handling, cooperative for exam. Patient exhibited excessive pain when attempting to do aural exam with otoscope. Allowed palpation of the ear pinnae, however resented deeper exam. Ear canals are severely thickened with a severe amount of black, ceruminous debris, AU. External canal appears mildly inflamed – unable to examine further without sedation A: 1. Severe otitis externa, likely secondary to stenotic canals Prognosis: Fair P: 1. Rec. Sedated ear exam, plus deep cleaning +/- short course of steroid (anti-inflammatory dose) 2. Start course of Tramadol 50 mg PO BID x 7 days.

Basic Information: Chipo is a 1 1/2 year old unaltered male Sharpei. He was given as a gift and has been in the same home since he was a puppy. Chipo is being surrender because the owner moved to a home that does not allow pets. Chipo has seen the vet regularly and has no known health problems or injuries. The owner says her favorite thing about Chipo is how friendly he is. Social Life and Personality: Chipo has been around children aged 18 months and older. He is relaxed and tolerant with the children but doesn’t usually play with them. He has lived with another dog his whole life and follows the other dog everywhere. Chipo is relaxed with the other dog and gentle during play. He has also lived with a cat and is relaxed and tolerant of the cat. Behavior: Chipo is not bothered when someone touches his food bowl while he is eating, removed from furniture, brushed, or woken up from sleep. He growls when another dog comes near his food bowl when he is eating. He struggles when given a bath. He is friendly when someone approaches his family. He growls if someone takes away a bone or rawhide. He barks at people that approach his yard. For a New Family to Know: Chipo knows how to sit. For exercise he was only allowed to play in the yard and he had free choice to go outside or come inside. He was not house broken and would use the bathroom inside daily despite having free choice of going outside. He was not given toys to play with. He was not crate trained. When left home alone he uses the bathroom indoors. He is well behaved when left in the yard alone. He is not bothered by loud noises such as fireworks or thunder. He was only fed boiled chicken and rice for meals. He was not given treats and he slept in bed with his owner. Intake Behavior: Chipo had a tense body during the intake and retreated when apparoched. He would move quickly when trying to collar. He relaxed enough when pet to allow the couneslor to collar him.

Helper Dog:A0935852 According to the owner Chipo has been around children ages 18 months and older, and was reported to be relaxed and tolerant with the children but doesn’t usually play with them. Chipo also has lived with another dog and cat and the owner stated he was gentle and respectful of them both. It was mentioned that Chipo is not bothered when someone touches his food bowl, but will growl when another dog comes near his food when he is eating. He was also reported to growl if someone takes away a bone or rawhide. At intake it was noted that Chipo had a tense body language and would retreat to the owners when approached, but when relaxed he allowed the counselor to pet and collar him. During SAFER Chipo was fearful, and needed time to feel comfortable enough to approach the assessor. He was tolerant of some handling, but when the assessor attempted the paw handling, He became stiff and lip curled. Due to this behavior and to avoid further escalation no other handling items were attempted. Chipo also became stiff and gave a low growl when being pushed out of the food bowl. He was not interested in the toy or rawhides items and had stiff body language when approaching the helper dog. In addition to the concerns seen on SAFER Chipo has also shown some concerns when on leash; mouthing the leash in attempts to getting away, and made several attempts to lunge at the assessor. Due to these behavior concerns placement with a New Hope partner is recommended. Look: 3. Dog will not allow Assessor to hold his head, and will not allow Assessor to conduct the assessment. Sensitivity: 2. Dog stands still and accepts the touch, his eyes are averted, his tail is down, body a bit tense, mouth closed, ears back. Tag: 2. Dog is fearful but unresponsive when touched. Dog approaches the Assessor when the game ends. (needed coaxing to approach.) Dog is crouching. Squeeze 1: 4. Dog lip curls. Food: 4. Dog freezes and growls. Toy 1: No interest. Rawhide: 1. No interest. Dog-dog: 3. Dog approaches the helper dog by rushing in with his tail high, stiff body, head tall, and ears erect.
