Medical Summary Scan : negative Sex: intact male Approx 5 yr S/O: BARH, mild conjuctivitis OU, no ND, dental disease 2/4. Mild skin scale/flakes. Multifocal areas of alopecia with broken hairs, suspect secodary to self-trauma and puritits related to fleas. Approx 6 cm firm mass on midline, top of head. Palpates like possible granuloma from blunt trauma injury. R front paw swollen, and mild lameness on right front limb. Non-painful on palpation, no open wounds or fractures palpated. Hematuria reported during intake. Abdomen palpated WNL, no bladder stones evident. A: Hematuria, right front limb lamness P: Start baytril x 14 days for suspect UTI, and Rimadyl PROGNOSIS: GOOD
