MEDICAL:
S/O: Vet check for dermatitis. Patient was surrendered with vet records from most recent exam. No work up ever performed – recommended to start novel protein diet. , mm – pink, moist CRT< 2 sec. , hydration – adequate, Integ- severe hyperpigmentation, alopecia, and lichenification over thoracic/abdominal region diffusely. Erythematous and alopecic over limbs and paws., EENT = clear, OU; thickened pinnae, lichenifcation, AU, nnd, moderate dental calculus, H/L = nma, nsr; pss; clear lung fields bilaterally, Abdomen = snp, no masses palpated, MS = amb x 4; BCS = 5/9, CNS = appropriate, LN = wnl
A:
1. Severe dermatitis – r/o infectious vs. inflammatory vs. underlying endocrinopathy vs. allergies vs. other
Prognosis: Fair
P:
1. Skin scrape tomorrow – if negative, sign up for dermachlor baths 2x weekly
2. Speak with New Hope – needs derm consult, full bloodwork performed.
Chewy has a severe skin condition that he has his whole life. He has severe hair loss and scratches ll the time. The vets have no figured out what the porblem is.
Basic Information:
Chewy is a 7 year old unaltered Shih Tzu. He was bought from a pet store as a puppy. He is being surrendered because the owner can no longer afford the medical cost for Chewy. He has a skin condition that he has had since a puppy. The owner says that her favorite things about Chewy is how lovable and playful.
Social Life and Personality:
Chewy has lived with children and he is relaxed and respectful of them. He is gentle during play with children. His owner says that when he is around other dogs he barks at them. They also say that he barks and chases cats. Chewy has been kept mostly indoors and has medium activity level. His owner describes him as playful, excitable, affectionate, anxious, independent, and friendly.
Behavior:
Chewy has bitten a dog in the past but the dog did not need any vet care. Chewy is not bothered if people touch his food bowl when is he is eating. He growls if someone takes away his bones, treats, or toys. He is friendly when he receives baths. He is afraid of having his nails trimmed. He is not bothered when brushed. He barks when strangers come to his door but is friendly when they come in the home. He will growl when he is woken.
For a New Family to Know:
Chewy knows how to come. His exercise consists of playing in the yard. He is let outside multiple times a day. He likes to play fetch with balls. He is crate trained and does well for 6 hours. He is trained to use the bathroom outside and on training pads or newspaper. When people are home he likes to follow them, be in the same room, or is lying in his favorite spots. He is well behaved when left home alone. When off leash he stays close by your side. He is scared of loud noises such as fireworks or storms. He panics and tries to hide. He is used to eating wet and dry dog food. The brand he is used to is Nutra. He sleeps in his dog crate.
Intake Behavior:
Chewy approached with a soft body. He started to wag his tail when talked to. He allowed handling without any aggressive behaviors. He remained relaxed and friendly throughout the intake.
BEAHVIOR:
When off leash with other dogs, Chewy is not interested in interacting with other dogs and will wander the yard. He offers an appropriate correction to a forward dog.
