Medical Summary Sex: Male neutered Scan: negative Hx: Pt surrendered by owner who has had him for 1 week only. Pt had dark runny diarrhea in the lobby. S: Active, alert, pays attention to everything, barks constantly, easy to handle, sits on command and takes treats when offered. O: BAR-H, BCS 2/9, MMs pink and moist, CRT <2 sec EENT: No discharge OU, nose. Mild debris AU. Moderate tartar on carnassial teeth; other teeth clean. PLNs: Not enlarged. H/L: NSR, NMA. Panting. Abd: Slightly tense, tucked up, no pain on palpation, no masses palpated M/S/I: Amb x4. No lesions noted. UG: Male neutered A: 1. Extremely underweight – R/O malnutrition vs. maldigestion (EPI) vs. GI parasites vs. other 2. Diarrhea in lobby – R/O primary GI problem vs. stress colitis 3. Tartar on carnassial teeth Short-term prognosis: Excellent P: 1. Cleaned ears during exam 2. Long-term veterinary care should include weight monitoring, fecal float and dewormers as necessary 3. Gave pyrantel 6 ml PO during exam 1088 weight 40.6
