scan negative intact male very thin – emaciated dull, yellow teeth coat has dandruff slightly red eyes ears have a high smell – does not like ears touched muzzle needed to be used during initial, kept tensing up and tried to bite when touching his ears weight: 77lbs dehydrated BAR good appetite for food seen NOSF

Initial 04/26: scan negative, intact male, very thin – emaciated, dull, yellow teeth, coat has dandruff, slightly red eyes, ears have a high smell – does not like ears touched, muzzle needed to be used during initial, kept tensing up and tried to bite when touching his ears, weight: 77lbs, dehydrated, BAR, good appetite for food seen, NOSF.
PER OWNER: Bush has been loosing weight steadly sine October 2014. Went to the vet in October and it was found he had worms in intestines. Was given meds and the worms went away a week later, but he he is still loosing weight. The vet told him that if the meds prescribed doesn’t work in bringing his weight back, it may be too late for him . No further diagonstics done, Owner says he is still bright and alert, no change in prsonality, NOT LETHERGIC. Stool is solid and urine is normal. Eating normal. fed once a day.
Re-exam 04/26
Hx: Came in today – having GI worms and weight loss
S: Attentive, calm, stands for exam. Shaking head during entire exam.
O: BAR-H, BCS 1/9 with severe muscle wasting over entire body, MMs pink and moist, CRT <2 sec
EENT: Medial entropion OU (worse OD), no discharge OU. Both ear canals filled up with black and red-tinged thick discharge, pt resists ear exam. Clean nose. Moderate tartar and gingivitis.
PLNs: Not enlarged.
H/L: NSR, NMA. Eupnic, quiet lung sounds.
Abd: Abd tucked up, very tense or painful on abd palp – pt jumps and moves away.
M/S/I: Amb x4. Oblong mass (3 cm long) on lateral aspect of 5th metatarsal – soft, movable, overlying skin is haired and erythematous. No other lesions noted.
UG: Male intact, testicles soft and symmetrical.
Ear swab for ear mites: no mites seen
HWT: negative.
CBC: all WNL. Eosinophils at higher end of normal range (0.96, n 0.06-1.23) but well within normal. Hct 44.1%, WBC 8.58, plt 251.
Chemistry: All within normal limits except for slightly elevated globulins (4.6, n 2.5-4.5). T protein 7.5, BUN 11, creat 1.5, liver enzymes at low end of normal range, lytes WNL.
A:
1. Severe amt of ear debris – R/O ear mites vs. otitis externa
2. Emaciated, muscle wasting – R/O not being fed enough vs. underlying medical condition – neoplasia, intestinal parasites, IBD, GI malabsorption, EPI, other. No sign of severe disease seen on bloodwork. Elevated globs may be spurious or represent mild infection, possibly GI parasites.
3. Mass on lateral aspect of right hind leg – R/O benign vs. malignant
4. Entropion
Short-term prognosis: Fair
P:
1. Pt is BAR-H, active. Major problems are being underweight and having otitis externa – both easily treated. Pt also has a small mass on right hind leg.
2. Ears cleaned during exam and Otibiotic tx started – continue for 10 days
3. Clean ears EOD x3 cleanings.
4. Feed 3x daily
5. Reweigh in 5 days, notify vet of new weight.
