Medical Summary Scanned positive with chip # 985121007337123 QARH – clearly uncomfortable, but responsive to voice and allows handling N~ 9 years Ears, eyes, nose mouth clear; left lower canine tooth protrudes over upper lip Alopecia over most of head and torso; bleeding scabs on head and neck. Skin scrape showed positive for Demodex. 0.2mL Ivermectin given. Nails long; right front dewclaw curved and penetrating pad; trimmed and cleaned with Nolvasan. Left front dewclaw broken. Matted fur around all four feet.
