Medical Summary Scan neg Owner has to surender d/t can not handle cost of more than routine vet care. . NH placement needed ASAP for workup/continuity with treatment. S/O: Presently at 64.8 mg Phenobarbital BID PO. Has been controlled for months but seizured last night and seizured in customer care area. After seizure, 0915 estimated wt and gave Keppra for first post ictal dose Appearance: Post-ictal at first, when into medical room QAR to BAR, Does not like the cage, barks for attention, then wants to roam the the room and solicit rubs and petting from all then naps. Integument: Appropriate hair coat, normal nails, small raised haired mass at L pinnae base – palpated like a skin tag or other benign mass, sebaceous cyst in R flank. EENT: OU clear, AU- dirty,yeasty, No nasal d/c Oral cavity: MM pink, CRT <2sec, Healthy dentition PLN: WNL H/L: No murmur or arrhythmias, Lungs clear ABD: Soft non painful, U/G: M/S: Amb x 4, overwt BCS – 6.5 /9 Neuro: NSF, veterinary history at practitioner with phenobarbital treated seizures NOSF A: Seizures Otitis externa AU R Flank cyst – can be removed with sx L ear base mass – can be removed – mostlikely benign but can be Bxied Overwt- will need management, phenobarb can add to excess BCS P: Phenobarb not available at ACC. Keppra 5.25 cc PO TID until leaves. It is Imperative that this gets done while at ACC! NH placement needed ASAP – For seizure maintenance and workup if does stabilize with current Keppra dose. Rescue will need vet to give Rx for Keppra for 1-3 months at a time. Can be regulated with private vet but if seizuring continues will need further diagnostic workup.
