Medical Summary Scan : negative Sex: neutered Aprrox 11 yr S/O: BARH, lenticular sclerosis OU, otitis externa, severe dental disease 4/4 and calculus buildup. Reverse sneezing or nasal congestion during exam. Suspect possible oronasal fistula secondary to severe dental disease A: Geriatric, dental disease, otitis externa P: Clindamycin, ear cleaning and mometamax x7 days Prognosis: FAIR

PROFILE:
Bamm Bamm is an 11 yrs. old black and white beagle who was surrendered at ACC today. Bamm Bamm is neutered and he will undergo a full physical health exam. He is described as being affectionate and playful.
Bamm Bamm has been socialized with children cats and dogs. When he is around children cats and doogs he is relaxed respectful curious and tolerant. When he is interacting withthem he will be gentle affectionate and well behaved. He is an indoor and outdoor dog who is house trained and well behaved in both enviornments. He is described as being playful confident excitable affectionate mellow independent and friendly. He has a medium activity level and he is afraid of loud noises of any kind.
Bamm Bamm is not bothered when anyone places their hand by his food or when anyone takes away his toys or treats. He is also not bothered when he is pushed off of the furniture held back or disturbed from his sleep. He is not bothered when he is given a bath or having his fur brushed. He has never had his nails trimmed. He will growl when anyone approches his family or home. He knows the commands sit down come and leave.
Bamm Bamm likes to go for slow walks and will use the bathroom on grass or cement. He likes to play with balls aand chew bones. He eats wet and dry food twice a day and he has dog biscuit and dental treats for snack. When he is in the house with his family he will follow them around and stay in the same room with them. He likes to stay in his favorite spot the bedroom and will sleep anywhere in the house.
During the Intake process Bamm Bamm was calm and well behaved. He engaged people on a limited basis sniffing them and walking away. He allowed himself to be collared and photographed with no issue. He also allowed himself to be walked by staff members.
