I came in the shelter as a STRAY on 10/26/2015 from NY 10472, owner surrender reason stated was STRAY.

10/29/2015 Exam Type CAGE EXAM – Medical Rating is 4 NC – SEVERE CONDITIONS NOT CONTAGIOUS, Behavior Rating is NONE, Weight 18.0 LBS.
10/29/15 12:20 vet check for lethargy BARH, approaching front of cage and looking for attention, no sign of v P: continue to monitor 10/28/15 19:16 vomiting and lethargy reported PE wnl but patient very quiet and chin stained with saliva gave 0.7ml (7mg) maropitant SQ recheck tomorrow 10/27 Hx: Concerns that pt is blind S: Active, comes toward examiner for attention, wags tail O: BAR-H, MMs pink and moist, BCS 3/9 EENT: OD: Pigment keratitis covering entire cornea, injected conjunctiva, moderate amts of dry sticky mucus. OS: neovascularization and conjunctiva are growing over entire cornea. Injected conjunctiva, severe amts of dry sticky mucus. Pt resists exam of left eye. AU: yellow waxy debris, mildly stenotic canals, epithelial hyperplasia. No discharge from nares. H/L: NSR, NMA. Eupnic, quiet lung sounds. Abd: Soft, no pain on palpation, no masses palpated M/S/I: Amb x4. No lesions noted. UG: Female Schirmer tear test: OD 10 mm/30 sec. OS: 3 mm/30 sec. A: 1. KCS OU 2. Otitis externa AU 3. Underweight Short-term prognosis: Fair P: 1. Cleaned eyes and applied Optimmune during exam. Start Optimmune BID for life. Recheck eyes in 5 days. 2. Tech check to clean ears and start Otibiotic (pt was losing patience with us after STT and eye cleaning so I cut exam/treatments short) 1088

10/26/15 13:28 Basic information Alyce was brought in as a stray. Client says Alyce was friendly and immediately allowed him to interact, so he brought him into the care center. Behavior during Intake Alyce was friendly during intake. He allowed all handling to be conducted and accepted treats. Alyce showed no signs of aggression during intake.

10/28/2015 BEHAVIOR EVALUATION – AVERAGE

Alyce walks calmly on leash. She was calm and relaxed during handling. Alyce was distracted, focused on objects in the room rather than the handler during the tag test. She was distracted, focused on objects in the room rather than the handler during the tag test. Alyce was a bit difficult to push out of the food bowl, but had soft body language. She was not interested in toys or rawhide. Alyce did not show any concern during the behavior assessment. The behavior department feels that she can go to an Average home. Look: 1. Dog holds gaze with soft eyes, soft body. She allows head to be held loosely in Assessor’s cupped hands. Dog holds gaze for three full seconds. Sensitivity: 1. Dog stands still and accepts the touch, her eyes are averted, and her tail is in neutral position with relaxed body posture. Tag: 2. Dog is not fearful, but is unresponsive to the Assessor, and approaches the Assessor at the end of the game (may need coaxing to approach.) she is focused on stimuli other than the Assessor. Squeeze 1: 1. Dog gently pulls back her paw. Squeeze 2: 1. Dog gently pulls back her paw. Food 2. Dog follows the dish with her tail down, body likely a bit stiff. Dog lifts head after a bit of pressure from hand to cheek. Toy 1: No interest. Rawhide: 1. No interest.
