Medical Behavior Evaluation BLUE
Medical Summary Scan : negative Sex: spayed S/O: BAR, APP ++, severe dental disease and calculus grade 4/4 dental disease. Dried and wet mucopurrulent ocular discharge OU, neovascularization and increased corneal opacity OU. OD has 2mm area of epithelialization ventromedial. Severe dark brown waxy discharge AU. Small pedunculated abdominal mass on midline, ~2cm. Palpates like connective tissue and fluid. Clipped nails, and cleaned ears, mild discomfort during ear cleaning. Did not attempt to visualize tympanic membrane. A: Presumptive KCS and otitis externa, otitis media could not be ruled out P: Start erythromycin x 7 days, begin KCS therapy w/ optimmune when finished. Would recommend placement w/ NH parter and more complete medical workup upon placement, suggest CBC/Chem/UA, ocular minimum database. Needs multiple dental extractions. PROGNOSIS: GOOD w/ follow-up veterinary care

Basic information
Tiny is a 7 year old spayed brown Chihuahua that is possible partially blind. Owner has had tiny for 7 years and is surrendering because of personal problems.
Socialization
Around strangers Tiny is relaxed and playful. Tiny lived with children ages 10,11,12 and got along well with them. Tiny plays gently with strangers adults and children. Tiny has not been socialized with other dogs or cats.
Behavior
During storms and fireworks tiny is relaxed. Tiny is tolerant of getting bathes and being brushed. When the Family touches Tiny’s food bowl she isn’t bothered by it but if a stranger goes to touch her food she will growl. When someone unfamiliar approaches the house Tiny will bark but upon entering will smell and greet them.
For the new family to know
Tiny was described as playful, and friendly with a medium activity level. When home with tiny she follows you around seeking attention. Tiny is mostly house trained and will sometimes have an accident when excited. Tiny is well behaved when left in the house alone. Tiny knows the commands sit and laydown. For exercise Tiny enjoys slow walks on the leash. When off leash Tiny stays close by. Tiny eats both wet and dry food twice a day and chicken a couple of times a week.
Behavior during intake
During intake Tiny was relaxed and quiet. When counselor approached Tiny smelled their hand and allowed to be pet. When trying to collar Tiny she became tense and attempted to bite. After a getting more use to the counselor Tiny allowed to be scanned collared and have her picture taken.
