Medical Summary 10/29/15 11:12 INITIAL EXAM microchip scan negative approx 10-12 year old male intact (2 descended testicles) terrier mix dog severely matted hair coat with urine and feces soaked hair, unknown if wounds underlying long nails heart murmur (3/6, left sided) coughing frequently on exam, slightly harsh lung sounds but no crackles ausculted severe dental disease – missing most of his teeth small (approx 2 cm in diameter) mass on right side of prepuce, infected with purulent discharge cataracts OU BCS 3/9, prominent spine ear canals have mild amount of brown discharge AU dried ocular discharge OU, no nasal discharge dermatitis suspect neglect PLAN: Give DA2PP vaccine SQ over right front leg Give rabies vaccine SQ over right hind leg Give CIV SQ over left hind leg Give bordetella vaccine IN Give pyrantel 1.4 ml Sign up for shave down, bath and activyl application Start clavamox 125 mg PO BID x 14 days for dermatitis and infected mass
