Medical Summary Scanned positive with chip # 985121009506789 QARH – tense, very sensitive around mouth, allows handling N~ 8 years Ears, eyes, nose, mouth clear Teeth stained Large soft mobile submandibular mass (“golf ball sized”) – no significant abnormalities to oral cavity Auscultate clear Skin rash face, ears, thorax, abdomen, genital area – moderate erythema and multiple epidermal collerettes – superficial pyoderma likely per dvm – Rx 0.42 Convenia given Dermachlor bath given A; Submandibular mass – r/o benign (lipoma vs sialocele vs. other) vs. malignant (thyroid carcinoma) – does not appear to impede on eating superficial pyoderma Px: Good short term. P: recommend 2x weekly baths and monitor skin recommend full work up of mass (FNA, chest x-rays, poss. U/S), blood work upon placement continue to monitor appetite, comfort
